雾化吸入氨溴索联合机械振动排痰对AECOPD患者疗效的观察

摘    要:目的探讨雾化吸入氨溴索联合机械振动排痰对慢性阻塞性肺疾病急性加重期(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)患者的临床效果。方法 AECOPD患者60例,随机分为对照组:氨溴索雾化组;实验组:氨溴索(30 mg,3/d)雾化治疗联合机械振动排痰组,每组各30例。两组患者均给予常规治疗。观察治疗前后两组患者日均排痰量、动脉血气指标[动脉氧分压(partial pressure of oxygen in artey,PaO2)和动脉血二氧化碳分压(partial pressure of carbon doxide in artey,PaCO2)]及肺功能指标[第1秒用力呼气容积(forced expiratory volume in one second,FEV1)/用力肺活量(forced vital capacity,FVC)(简称一秒率FEV1/FVC%)和FEV1],比较两组患者血白细胞恢复正常天数及住院总天数的差异。结果实验组前3天日均排痰量较对照组比较明显增多(P〈0.05),第4~7天日均排痰量则较对照组比较明显减少(P〈0.05);实验组第3天血气指标PaO2和PaCO2、1周后肺功能指标(FEV1/FVC%和FEV1)及血白细胞恢复正常天数、住院总天数较对照组均明显改善(均P〈0.05)。结论雾化吸入氨溴索联合机械振动排痰对AECOPD患者促排痰效果更显著,能改善肺通气及氧合功能,减轻炎症反应,明显缩短住院时间。

Abstract:Objective To evaluate the effects of ambroxol atomization combined with mechanical assistance on pa- tients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD).Methods Based on the standard of "Global Initiative for Chronic Obstructive Lung Disease (GOLD)", 60 AECOPD patients were randomly divided into con- trol group treated with amhroxol atomization and experimental group treated with ambroxol atomization combined with mechanical assistance. Both groups accepted routine therapies. The average expulsion of phlegm after treatments, the PaO2 ,PaCO2 and FEV1/FVC%,FEV1, the days of hemogram recovery and hospitalization were compared between two groups.Result The average expulsion of phlegm in experimental group was significantly increased compared to that in control group on day 1 to day 3 (P〈0.05) ,and the condition was converse on day 4 to day 7 (P〈0.05).The blood PaO2 and PaCO2 on day 3, FEV1/FVC% and FEV1 on day 7, the days of hemogram recovery and hospitalization after treat ments in experimental group were apparently improved compared to that in control group (P〈0.05).Conclusion Ambr- oxol atomization combined with mechanical assistance can significantly increase the average expulsion of phlegm in AE- COPD patients, improve their lung functions of ventilation and oxygenation, inhibit the lung inflammation, and ultimately decrease the hospitalization days.
