三种内固定方法治疗髌骨横行骨折的临床研究

目的:比较3种内固定方法治疗髌骨横行骨折的临床疗效.方法:回顾性分析髌骨横行骨折56例,分为空心加压螺丝钉组16例(A组),克氏针加钢丝张力带组18例(B组),微创Cable-pin组22例(C组).随访6~18个月,比较3组术后膝关节Lowa评分、手术时间、骨折愈合情况、手术费用和术后并发症.结果:3组患者膝关节Lowa评分差异无统计学意义(P>0.05).A组和C组的手术时间与手术费用均明显多于B组(P<0.01);而C组的骨折愈合时间、术后并发症均明显少于A组和B组(P<0.01).结论:微创Cable-pin钉虽然价格昂贵,但组织激惹少、愈合时间短、术后并发症少,值得临床推广.

The clinical study of three kinds of internal fixation methods in the treatment of the patellar transverse fracture

Objective:To compare the clinical effects of three kinds of internal fixation methods in the treatment of patellar transverse fracture. Methods:The clinical data of 56 patients with patellar transverse fractures were retrospectively analyzed. Sixteen cases were treated with the hollow compression screw( group A) ,18 cases were treated with Kirschner needle combined with steel wire tension band ( group B) and 22 cases were treated with minimally invasive Cable-pin nail( group C) . All cases were followed up for 6 to 18 months. The postoperative knee joint lowa score, operation time, postoperative fracture healing, operation cost and postoperative complications between three groups were compared. Results:The differences of the knee joint lowa scores between three groups were not statistically significant(P>0. 05). The operation time and cost in group A and group C were significantly higher than those in group B(P<0. 01). The fracture healing time and postoperative complications in group C were significantly less than those in group A and group B ( P<0. 01). Conclusions:The minimally invasive Cable-pin nail treatment is the least tissue irritation, short healing time and less postoperative complications,which is worthy of promotion in clinic.
