CEA、CYFRA21-1、NSE联合检测对于肺癌早期诊断的临床价值

摘    要:目的 探讨癌胚抗原(CEA)、血清角蛋白19片段(CYFRA21-1)、神经源特异性烯醇化酶(NSE)联合检测对于肺癌早期诊断的临床价值和效果分析,为肺癌的早期诊断提供科学依据.方法 回顾性分析200例肺疾病患者的临床资料,其中,100例肺癌患者作为病例组,100例肺部良性疾病患者作为对照组.收集患者的年龄、性别、体质量指数(BMI)等一般人口学特征资料,并对所有研究对象使用实验室电化学发光法检测CEA、CY-FRA21-1和NSE.结果 研究对象的平均年龄为(54.68±4.89)岁;其中,男性127例,占63.5%,女性73例,占36.5%;经ROC分析发现,对于CEA而言,截断点为3.52,灵敏度为81.8%,特异度为80.2%;对于CYFRA21-1而言,曲线下面积为0.746,灵敏度为79.8%,特异度为80.7%;对于NSE而言,曲线下面积为0.789,灵敏度为79.2%,特异度为80.4%.联合CEA、CYFRA21-1、NSE发现,曲线下面积为0.934,灵敏度为91.25%,特异度为94.34%.结论 CEA、CYFRA21-1和NSE联合检测对于早期肺癌具有较高的诊断价值,可以显著提高肺癌早期诊断的灵敏度和特异度.

Abstract:Objective To investigate the clinical value and effect of combined detection of carcinoembryonic antigen (CEA),serum keratin 19 fragment(CYFRA21-1)and neuron specific enolase(NSE)in the early diagnosis of lung cancer, to provide a scientific basis for the early diagnosis.Method A retrospective study concerning 200 cases of lung disease was conducted,in which 100 lung cancer patients were assigned as the study group,and another 100 patients with benign lung disease were control group.The general demographic characteristics of patients,such as age,sex,and BMI,were col-lected,and all subjects were administered with laboratory electrochemical luminescence detection of CEA,CYFRA21-1 and NSE.Result The mean age of the subjects was 54.68±4.89,of which 127 cases(63.5%)were male and 73 cases (36.5%)were female.In ROC analysis,for CEA,the cutoff point was 3.52,with sensitivity being 81.8% and the specifici-ty was 80.2%;for CYFRA21-1,the AUC was 0.746,the sensitivity was 79.8%,and the specificity was 80.7%;for NSE, the AUC was 0.789,the sensitivity was 79.2%,and the specificity was 80.4%.Combined detection of CEA,CYFRA21-1, and NSE was observed with an AUC of 0.934,of which the sensitivity and specificity were 91.25% and 94.34%,respec-tively.Conclusion The combined detection of CEA,CYFRA21-1 and NSE is of higher diagnostic value for early diag-nosis of lung cancer,and can significantly improve the sensitivity and specificity of early diagnosis.
