彩色多普勒超声血流成像联合超声弹性成像对乳腺肿块的定性诊断价值

摘    要:目的:探讨彩色多普勒血流成像联合超声弹性成像在乳腺肿块定性诊断中的价值.方法:对156例乳腺肿块患者(174个肿块)分别进行彩色多普勒血流成像和超声弹性成像,并与病理检查相对照,比较2种方法联合检测和分别单独检测诊断乳腺恶性肿瘤的敏感度、特异度和准确度.结果:彩色多普勒血流成像诊断乳腺恶性肿瘤的敏感度、特异度和准确度分别为82.1%、72.6%和78.7%,超声弹性成像分别为84.8%、74.4%和80.5%,二者联合分别为88.4%、91.9%和89.7%.乳腺肿块彩色多普勒血流成像联合超声弹性成像与病理检查结果差异无统计学意义(P>0.05).结论:彩色多普勒血流成像联合超声弹性成像可对乳腺良恶性肿块进行有效的定性诊断,是乳腺疾病早期诊断的有效手段.

Abstract:Objective:To explore the qualitative diagnosis value of the color Doppler ultrasound combined with ultrasonic elastography on breast masses. Methods:One hundred and fifty-six patients with breast mass(174 masses) were detected using the color Doppler flow imaging combined with ultrasonic elastography, the result of which was compared with pathological findings. The sensitivity, specificity and accuracy in diagnosing malignant breast tumor between the combined detection of two methods and single method detection were compared. Results:The sensitivity,specificity and accuracy of the color Doppler ultrasound in the diagnosis of malignant breast tumor were 82. 1%,72. 6% and 78. 7%,respectively. The sensitivity,specificity and accuracy of ultrasonic elastography in the diagnosis of malignant breast tumor were 84. 8%,74. 4% and 80. 5%,respectively. The sensitivity,specificity and accuracy of the color Doppler ultrasound combined with ultrasonic elastography in the diagnosis of malignant breast tumor were 88. 4%,91. 9% and 89. 7%, respectively. The difference of the detection result between the color Doppler ultrasound combined with ultrasonic elastography and pathological examination was not statistically significant(P>0. 05). Conclusions:The detection of color Doppler ultrasound combined with ultrasonic elastography can effectively diagnose the malignant breast masses,which is an effective mean in early diagnosis of breast disease.
