摘    要:目的:探讨甘草酸二铵对腹膜透析的腹膜纤维化大鼠腹膜结缔组织生长因子(CTGF)的影响.方法:将50只SD大鼠随机分成空白对照组、0.9%氯化钠注射液组、模型组、已酮可可碱治疗组、甘草酸二铵治疗组,各10只,建立大鼠腹膜透析模型,己酮可可碱治疗组同时给予己酮可可碱,甘草酸二铵治疗组同时给予甘草酸二铵进行干预,连续28 d,于第30天行4 h腹膜平衡实验测定腹膜功能,并留取壁层腹膜,观察病理形态,测量腹膜厚度,检测腹膜组织CTGF的表达.结果:光镜下,HE染色可见空白对照组和0.9%氯化钠注射液组腹膜表面覆盖着一层扁平间皮细胞,模型组、已酮可可碱治疗组和甘草酸二铵治疗组显示腹膜间皮细胞变形成为柱状、圆形,间皮细胞可见部分脱落,基质增厚,发现大量成纤维样细胞及巨噬、单核细胞浸润,且有纤维素样物质沉积.Masson染色显示有明显的胶原沉积,模型组比已酮可可碱治疗组和甘草酸二铵治疗组胶原沉积更明显.各组大鼠腹膜厚度为:模型组>已酮可可碱治疗组、甘草酸二铵治疗组>空白对照组、0.9%氯化钠注射液组(P<0.01);各组腹膜超滤量和D4/D0为:空白对照组、0.9%氯化钠注射液组>已酮可可碱治疗组、甘草酸二铵治疗组>模型组(P<0.01);各组腹膜Durea/Purea值和CTGF表达水平为:模型组>已酮可可碱治疗组、甘草酸二铵治疗组>空白对照组、0.9%氯化钠注射液组(P<0.01).结论:4.25%腹膜透析液会使大鼠腹膜纤维化,甘草酸二铵可以减轻腹膜纤维化及腹膜功能恶化.

Abstract:Objective:To investigate the effects of diammonium glycyrrhizinate on peritoneal connective tissue growth factor(CTGF) in retroperitoneal fibrosis rats treated with peritoneal dialysis.Methods:Fifty male SD rats were randomly divided into the control group,NS group,model group,pentoxifylline group and diammonium glycyrrhizinate group(10 rats each group).The peritoneal dialysis model of rats was established.The pentoxifylline group and diammonium glycyrrhizinate group were treated with pentoxifylline and diammonium glycyrrhizinate for 28 days,respectively.On the 30th day,the 4 h peritoneal equilibration test was adopted to determine the rats′peritoneum function,the partial peritoneum tissue of rats was collected to observe the pathological morphology and thickness,and the expression of CTGF in peritoneal tissue was detected.Results:The HE staining showed that a layer of flattened cells covered the peritoneum surface were found in control group and NS group,and the column and round cells,abstraction,matrix thickening,a large number of fibroblast-like cells,monocytes and macrophages infiltration,and fibrinoid material deposition in peritoneal mesothelial cells were found in model group,pentoxifylline group and diammonium glycyrrhizinate group.Masson staining showed that the apparent collagen deposition was found,which in model group was more than that in pentoxifylline and diammonium glycyrrhizinate group.The peritoneum thickness in model group,pentoxifylline treatment group,diammonium glycyrrhizinate treatment group,control group and NS group gradually decreased(P<0.01).The peritoneal ultrafiltration and D4/D0 in control group and NS group pentoxifylline group,diammonium glycyrrhizinate group and model group gradually decreased(P<0.01).The value of Durea/Purea and CTGF level of peritoneum in model group,pentoxifylline group,diammonium glycyrrhizinate group,control group and NS group gradually decreased(P<0.01).Conclusions:The 4.25% peritoneal dialysis solution can lead to peritoneal fibrosis in rats,and the diammonium glycyrrhizinate can alleviate peritoneal the peritoneal fibrosis and function.
