围手术期综合呼吸功能训练对老年患者腹腔镜术后肺功能的影响

目的:探讨围手术期综合呼吸功能训练对老年患者腹腔镜术后肺功能的影响.方法:选择接受腹腔镜下阑尾切除术及胆囊切除术的73例老年患者,随机分为对照组36例和观察组37例,分别进行常规护理及呼吸功能训练.对2组患者术后并发症、术前及术后肺功能、血气分析进行比较.结果:观察组各并发症发生率与对照组差异均无统计学意义(P>0.05).对照组术后FEV1、FEV1/FEV较术前下降(P<0.05),观察组术后FEV1、FEV1/FEV较对照组均有改善(P<0.05).对照组术后SaO2、PaO2较术前下降,PaCO2升高(P<0.05),观察组术后SaO2、PaO2、PaCO2较对照组均有改善(P<0.05).结论:围手术期综合呼吸功能训练是改善老年患者腹腔镜术后呼吸功能的重要方法,并有助于减少术后呼吸系统并发症的发生.

Effect of perioperative comprehensive respiratory function training on the postoperative lung founction after abdominal laparoscopy surgery in elderly patients

Objective:To investigate the effects of perioperative comprehensive respiratory function training on postoperative lung founction after abdominal surgery in elderly patients. Methods:Seventy-three elderly patients treated with laparoscopic appendectomy or cholecystectomy were randomly divided into the control group ( 36 cases ) and observation group ( 37 cases ) . The control group and observation group were treated with the routine nursing and respiratory function training,respectively. The postoperative complication, preoperative and postoperative lung function and blood gas between two groups were compared. Results:The difference of the incidence rate of complication between two grouops was not statistically significnat(P>0. 05). Compared before operation,the postoperative FEV1 and FEV1/FEV in control group were decreased(P<0. 05),and the postoperative FEV1 and FEV1/FEV in observation group were improved compared with the control group(P <0. 05). Compared before operation,the postoperative SaO2 and PaO2 significantly decreased,and the postoperative PaCO2 increased in control group(P <0. 05). Compared with the control group,the postoperative SaO2,PaO2 and PaCO2 in observation group were improved(P <0. 05). Conclusions:The perioperative comprehensive respiratory function training is an important way in improving respiratory function after laparoscopic surgery,which can help to reduce the incidence of postoperative respiratory complications.
