摘    要:目的 探讨胃癌化疗患者营养状况与化疗不良反应、疗效及生活质量的关系.方法 对80例胃癌化疗患者应用营养风险筛查量表(NRS2002)评估其营养状况,并根据评估结果将患者分为无营养风险组(NRS2002<3分,n=42)及营养风险组(NRS2002≥3分,n=38),比较两组患者化疗不良反应、近期治疗效果及生活质量情况.结果 无营养风险组前白蛋白、转铁蛋白、血红蛋白、淋巴细胞计数水平明显高于营养风险组(P﹤0.01);无营养风险组胃肠道、血液系统、神经系统及骨髓抑制不良反应发生情况低于营养风险组(P﹤0.05);无营养风险组近期治疗总有效率高于营养风险组(P﹤0.05);无营养风险组患者社会功能、情绪功能、认知功能、角色功能、躯体功能5个维度评分及总生活质量评分明显高于营养风险组(P﹤0.01).结论 营养不良状况可能增加胃癌化疗患者不良反应,从而影响患者化疗效果及生活质量,因此化疗期间应加强患者营养干预.

Abstract:Objective To investigate the relationship between nutritional status, chemotherapy side effects, efficacy and quality of life in patients with gastric cancer. Method 80 cases of patients with gastric cancer were assessed by nutri-tional risk screening scale (NRS2002) to evaluate their nutritional status, and these patients were stratified as"without nu-tritional risk" (n=42, NRS2002<3 points) or "with nutritional risk" (n=38, NRS2002≥3 points). The side effects, short-term efficacy and quality of life of two groups were compared. Result The levels of prealbumin, transferrin, hemoglobin and lymphocyte count in patients without nutritional risk were significantly higher than those with nutritional risk (P<0.01);the incidence of side effects such as gastrointestinal tract, blood system, nervous system and bone marrow suppres-sion was lower (P<0.05), and the overall response rate of was higher (P<0.05), besides, the scores of social function, emo-tional function, cognitive function, role function and somatic function were significantly higher in patients without nutri-tional risk compared to those with nutritional risk (P<0.01). Conclusion Malnutrition increases the incidence of adverse reactions in gastric cancer patients treated with chemotherapy, thus affecting the efficacy of chemotherapy and the quality of life of patients, so nutritional interventions during chemotherapy should be strengthened.
