彩色多普勒能量图在肾动脉狭窄中的诊断价值

目的评价彩色多普勒能量图诊断肾动脉狭窄的临床价值。方法患者20例,其中确诊肾动脉狭窄患者16例。所有患者均接受彩色多普勒超声（CDFI）与能量多普勒（CDE）两种检查方法。诊断结果分3级：①不能得到诊断结果;②可疑诊断;③诊断明确。检查结果与数字减影血管造影结果相对照。结果常规彩色多普勒超声（CDFI）诊断的敏感性为56.3%（9/16）,特异性为50.0%（2/4）,彩色多普勒能量图（CDE）的敏感性为75.0%（12/16）,特异性为75.0%（3/4）。结论 CDE是一种较敏感的彩色多普勒技术,可以显示CDFI难以探测到的低速血流,且具有相对的非角度依赖性,因此对肾动脉总体显示率高于CDFI,从而有效的提高了肾动脉狭窄的检出率。

Clinical value of color Doppler energy in the diagnosis of renal artery stenosis

Objective To evaluate the diagnosis value of color Doppler energy in the diagnosis of renal artery stenosis.Methods 16 patients with renal artery stenosis were examined with conventional color Doppler sonography and color Doppler energy.The diagnostic results were divided into three scales:no diagnosis result,doubted diagnosis;unequivocal diagnosis.The results were compared with those of digital subtraction angiography.Results The sensitivity,specificity of conventional color Doppler scanning were 56.3%(9/16),50%(2/4),respectively;those of color Doppler energy imaging were 75%(12/16),75%(3/4),respectively.Conclusion Color Energy Doppler(CDE) is one kind of sensitive colored Doppler technology,may demonstrate low speed blood stream which CDFI surveys with difficulty,and has the relative non-angle dependence,therefore is higher than CDFI to the renal artery stenosis rate,thus can effectively improve renal artery narrow detection rate.
