前交叉韧带重建术后患者心理状态的临床研究

目的：探讨前交叉韧带重建术后患者的个性特征和临床心理症状.方法：采用艾森克个性问卷（EPQ）、症状自评量表（SCL-90）、汉密顿抑郁量表（HAMD）、汉密顿焦虑量表（HAMA）对42例前交叉韧带重建术后患者进行评定,并与30名健康对照者比较.结果：①前交叉韧带重建术后组和健康对照组EPQ的N维度分值分别为（55±11）分、（45±11）分,P维度分值分别为（55±11）分、（49±9）分,两组比较差异有统计学意义（P均＜0.05）.②SCL-90发现前交叉韧带重建术后组躯体化（1.71±0.60）、抑郁（1.98±0.51）、焦虑（1.89±0.66）、恐惧（1.59±0.58）、偏执（1.79±0.63）、精神病（1.64±0.48）和其他（1.98±0.41）分值均明显高于对照组（P＜0.01或0.05）.③前交叉韧带重建术后组有焦虑症状者38例（90%）,有抑郁症状者36例（86%）.结论：前交叉韧带重建术后患者情绪不稳定、敏感、多疑,容易焦虑和抑郁,提示心理干预对其康复可能有重要意义.

Clinical study on psychological status of the patients with ACL reconstruction

Objective： To study the personality trait and psychological status of patients with ACL reconstruction. Methods： EPQ, SCL-90, HAMD and HAMA were performed in 42 patients with ACL reconstruction and 30 health subjects. Results： ① N and P scores in EPQ in the patients with ACL reconstruction and healthy controls were 55 ± 11 and 55 ± 11, and 45 ± 11 and 49 ± 9, respectively. Significant differences were observed in N and P scores between the two groups （P 〈 0. 05）. ② SCL-90 revealed that somatization, anxiety, depression, phobic anxiety, paranoid ideation and psychoticism were significantly more prominent in patients with ACL reconstruction （P 〈0. 05）. ③ Anxiety and depression were presented in 38 （90%） and 36 （86%） patients with ACL reconstruction, respectively. Conclusion： Unstable emotion, sensitiveness, suspiciousness, anxiety and depression are presented in patients with ACL reconstruction, thus providing psychological intervention in rehabilitation patients is necessary.
