膝关节镜下前交叉韧带损伤的解剖特点与临床效果分析

目的总结前交叉韧带损伤的解剖特点诊断方法,探讨膝关节镜下治疗的临床效果,比较两种不同移植材料的临床疗效。方法选取2009年01月至2013年04月单纯前交叉韧带损伤患者62例,患者均行关节镜下重建手术,按不同移植材料分为两组：自体骨-髌韧带-骨组（A组）32例;自体半腱肌腱股薄肌腱组（B组）30例。术后定期随访2年,对Wemer髌股痛评分、肌力恢复程度、膝关节活动度、国际膝关节文献委员会评分、Lysholm评分等进行观察对比。结果 62例患者均未发生严重并发症。术后2年两组比较膝关节活动度、肌力恢复情况、Lysholm膝关节评分、髌股痛评分、国际膝关节文献委员会评分差异无统计学意义（P〉0.O5）。结论前交叉韧带解剖结构复杂,膝关节镜下前交叉韧带重建术后的疗效与移植材料的种类之间无明显关系。

Anatomical Characteristics and Clinical Effects of Arthroscopic Anterior Cruciate Ligament Injury

Objective To summarize the anatomical characteristics and diagnostic Methods of anterior cruciate ligament injury,and to explore the clinical effects of arthroscopic treatment,and compare clinical efficacy of the two different graft material.Methods From January 2009 to April 2013,62 cases with the simple anterior cruciate ligament injury all underwent arthroscopic reconstructive surgery,who were divided into two groups according to graft material：Autologous bone-patellar tendon-bone group（A） 32 cases ;Semitendinosus tendon gracilis tendon group（B） 30 cases.Regular follow-up after 2 years,the Wemer patellofemoral pain rating,degree of muscle recovery,knee activity,International Knee Documentation Committee score,Lysholm score were observed and compared.Results 62 patients had no serious complications.2 years later,the knee activity,muscle strength recovery,Lysholm knee score,patellofemoral pain score,International Knee Documentation Committee score of the two groups had no significant difference（P＆gt; 0.05）.Conclusion Anterior cruciate has ligament complex anatomical structure,there is no obvious relationship between the types of anterior cruciate ligament reconstruction with grafting material effect of postoperative knee under arthroscopy.
