布地奈德在治疗新生儿肺炎中的应用探讨

目的探讨布地奈德混悬液加压雾化吸入在佐治新生儿出生后感染性肺炎中的应用。方法将新生儿出生后感染性肺炎178例，随机分成两组，基础治疗相同，对照组采用生理盐水雾化吸入，每次20mL，每天2－6次；实验组加用普米克令舒经空气加压雾化吸入，每次0．25mL（含布地奈德125μg），每天2～3次。观察记录鼻阻消失时间、气促消失时间、肺部啰音消失时间、总有效率及鹅口疮发生情况。结果实验组88例，对照组90例，两组鼻阻消失时间分别为（8．2±2．4）d和（9．8±2．9）d，气促消失时间分别为（4．3±2．1）d和（5．6±2．6）d，肺部啰音消失时间分别为（5．5±1．7）d和（6．6±2．0）d（P〈0.01），总有效率分别为93．18％和82．22％（P〈0．05），鹅口疮发生率分别为9．09％和7．78％（P〉0．05）。结论在新生儿出生后感染性肺炎治疗中，加用布地奈德混悬液空气压缩雾化吸入，效果显著，使用安全，临床实用，值得推广。

The Application of Budesonide in Pneumonia of Newborn

Objective To discuss the application of budesonide suspension and inhalation after compression in assistant treatment of newborn pneumonia. Methods 178 neonates that suffered infectious pneumonia after bebomd were divided randomly into two groups, same regular treatments,contrastive group received 0.9% NS atomizing inhalation, experiment group received pulmicort respules atomizing inhalation after compression. Observe and record the time of rhinostegnosis disappeard, anhelation disappeard,rale in bellows disappeard, the rate of availability and thrush. Results The experiment group （n=88） and the contrastive group （n=90）, the time of rhinostegnosis disappeard respectively is（8.2 ± 2.4）d and（9.8± 2.9）d, the time of anhelation disappeard respectively is （4.3± 2.1 ）d and （5.6 ± 2.6）d,the time of rale in bellows disappeard respectively is （5.5 ± 1.7）d and （6.6 ± 2.0）d （P 〈 0.01 ）the rate of availability respectively is 93.18% and 82.22%（P〈0.05）, the rate of thrush respectively is 9.09% and 7.78%（P〉0.05）. Conclusion The method of using pulmicort respules atomizing inhalation after air prelum to assistant treat the neonates that sufferde infectious pneumonia after bebomd is useful,safe and simple,be worth of clinical expansion.
