新疆维吾尔族骨折患者血浆同型半胱氨酸水平与骨折愈合时间

摘    要:背景:高同型半胱氨酸血症(HHCY)是一个广泛的代谢问题,被认为是心血管疾病与骨质疏松症的重要危险因素。目的:探讨维吾尔族骨折患者同型半胱氨酸(HCY)水平及高HCY血症与骨折愈合时间的关系。 方法:收集366例四肢长骨骨折患者,其中汉族154例,维吾尔族212例;男204例,女162例。检测血浆HCY浓度,分析维吾尔族和汉族HCY水平差异及HCY水平对维吾尔族患者骨折愈合时间的影响。 结果:维吾尔族和汉族骨折患者在年龄、性别、职业、吸烟、骨折部位、骨折类型、骨折复位时间、骨折原因、有无合并伤、固定方式、术后康复依从性等方面无显著统计学差异(P〉0.05)。维吾尔族患者的血浆HCY水平、高HCY血症患病率显著高于汉族患者(P=0.000);维吾尔族患者平均骨折愈合时间长于汉族患者(P=0.000)。在维吾尔族患者中,高HCY组患者平均骨折愈合时间长于正常HCY者(P=0.000);经校正年龄、性别、职业、吸烟、骨折部位、骨折类型、骨折复位时间、骨折原因、有无合并伤、固定方式、术后康复依从性等因素后,维吾尔族患者骨折愈合时间与血浆HCY水平呈线性相关(r=0.784,P=0.000)。 结论:维吾尔族骨折患者血浆HCY水平高于汉族患者,且骨折愈合时间长于汉族骨折患者;维吾尔族患者中,血浆HCY水平增高可能是影响患者骨折愈合时间的重要因素。

Abstract:Background:High homocysteine (HHCY) is a broad metabolic problem and considered to be an important risk factor for car-diovascular disease and osteoporosis. 〈br〉 Objective:To investigate the relationship between fractures healing time and homocysteine level in Uygur fractured patients. 〈br〉 Methods:A total of 366 patients (including 154 Han and 212 Uygur subjects, 204 males and 162 females) with long bone fractures of extremities were selected as subjects. Plasma homocysteine level was measured and compared between Uygur and Han patients. Further, the relationship between fractures healing time and homocysteine was analyzed. 〈br〉 Results:There were significant differences in age, gender, occupation, smoking, fracture site, fracture type, fracture reduc-tion time, causes of fracture, associated injuries, fracture fixation methods, and postoperative rehabilitation training compli-ance between the Uygur and Han fractured patients (P>0.05). The plasma homocysteine concentration, the prevalence of hy-perhomocysteinemia and fracture healing time were significantly higher in Uygur patients than that in the Han individuals (P=0.000). The fracture healing time was higher in patients with hyperhomocysteinemia than individuals without hyperho-mocysteinemia (P=0.000). After adjusting for age, gender, occupation, smoking, fracture site, fracture type, fracture reduc-tion time, causes of fracture, associated injuries, fracture fixation methods, and postoperative rehabilitation training compli-ance, the fracture healing time was linearly correlated with plasma homocysteine level in Uygur patients (r=0.784, P=0.000). 〈br〉 Conclusions:In the study, the homocysteine level in Uygur is higher than that in Han patients with extremities fractures, and fracture healing time in Uygur is longer than that in Han patients. The increased plasma homocysteine concentration may be the important factor influencing the fracture healing in Uygur fractured patients.
