超选择子宫动脉栓塞治疗产后大出血

目的：探讨超选择子宫动脉栓塞治疗产后大出血的疗效及安全性。方法：临床15例产后大出血患者采用右股动脉Seldinger穿刺技术,超选择插管至双侧子宫动脉,行DSA明确出血部位后,行介入栓塞。结果：插管成功率100%,15例大出血病例栓塞后活动性出血停止,手术成功,1～5 d后阴道内完全无流血。结论：子宫动脉栓塞创伤小,疗效快速肯定,术后短期及长期随访无严重并发症,对危及生命的产后出血是一种有效治疗措施。

Superselective Uterine Artery Embolization:an Effective Treatment for Massive Postpartum Hemorrhage

Objective：To discuss the effect and safety of superselective uterine artery embolization for managing massive intractable postpartum hemorrhage.Methods： 15 patients with intractable postpartum hemorrhage were undertaken superselective catheterization into the bilateral uterial arteries to find the case and site of bleeding through DSA and then followed by arterial embolization.Result：The successful rate of catheterization was 100%.The achievement in 15 cases showed no active hemorrhage immediately after the procedure and no vaginal bleeding after 1～5 d ays.Conclusions： Superselective uterine artery embolization is a safe and effective means for control of intactable postpartum hemorrhage,providing less trauma and no severe complications,especially as an unique management for fetal postpartum hemorrhage.
