| 摘 要： | 采用LINUX系统的运行环境和JAVA＋ORACLE＋STRUTS的开发框架，设计并开发了一个包括社区双向转诊、复查提醒、健康咨询、健康指导、个人健康档案、会员活动、健康刊物、短信平台、系统管理等功能模块的出院后健康管理服务平台，旨在建立规范有效的公众健康服务体系，提供健康咨询和健康指导等服务，解决患者诊疗前的知识普及、出院后的跟踪服务，力求充分利用有限的医疗资源来达到最大的健康管理效果。

| Abstract: | We design and develop an post hospital discharge health management services platform with the operating environment of LINUX system and the development framework of JAVA＋ORACLE＋STRUTS, including many function modules just like communal bothway referral , review reminders, health advice, health guidance, personal health records, member activities, health publication, messaging platform, system management. This platform aims to establish a standardized and effective public health service system, to provide services such as health counseling and health guidance ,to achieve transmission of information before treatment and after sevice. In order to make full use of limited medical resources and strive to achieve largest health management effectiveness. |
