超声产前诊断双胎反向动脉灌注的价值

目的:探讨超声产前诊断双胎反向动脉灌注的价值。方法:回顾性分析5例双胎反向动脉灌注儿产前超声检查,包括绒毛膜囊数目、羊膜囊数目、胎盘位置、脐带胎盘插入口、胎儿生长测量指标、羊水最大深度、脐动脉血流动力学参数等,并与病理结果相对照。结果:超声表现均为单绒毛膜单羊膜腔,一胎儿形态结构正常,另一胎儿为无心畸胎,脐血管为一条脐动脉一条脐静脉,血流方向与正常反向。结论:产前超声检查对于双胎反向动脉灌注的尽早期明确诊断、预后的判断、进一步治疗方案的选择有重要意义。

The value of ultrasound in the diagnosis of twins reversed arterial perfusion

Objective：To evaluate the efficiency of the ultrasound examination of reversed arterial perfusion and the monitoring of fetal condition for twins during intrauterine.Methods：We studied five pairs of twins by retrospective analysis.In all cases reversed arterial perfusion was examined by ultrasound,and the number of human chorionic sac,amniotic sac number,location of the placenta and umbilical cord insertion placenta entrance,the index of fetal growth measure,maximum depth of amniotic fluid,umbilical artery blood flow parameters were calculated.The ultrasonographic manifestations of all cases were compared with its pathological results.Results：The results of ultrasonography on the cases are single chorionic single amniotic cavity,a normal fetal morphology,and the other acardiac twins,an umbilical artery cord blood tube,a human umbilical vein,and blood enters the acardiac/ acephalic twin through reversed flow through its umbilical artery.Conclusion：Five cases of twins reversed arterial perfusion（TRAP） sequence with relevant ultrasound features that would help an accurate diagnosis and prognosis and further treatment options.It is very important to monitoring of fetal condition for twins during intrauterine by the ultrasound examination of reversed arterial perfusion.
