58例肝脏局灶性结节增生的回顾性分析

目的探讨肝脏局灶性结节增生(focal nodular hyperplasia,FNH)的诊断和治疗经验。方法回顾性分析我院1996年4月至2002年3月收治并经手术证实的58例FNH病人的临床、病理学和影像学资料。结果多数病人为男性(男/女3.5/1.0)且无口服避孕药史(54/58);大多数病人无临床症状,肝功正常;多期扫描螺旋CT(13/18)和MRI(11/16)有特征性表现。无手术死亡。结论FNH在螺旋CT与MRI的动态增强扫描具有特征性表现;对于诊断明确且无临床表现者,可定期影像学观察。手术适应证为:(1)有明显压迫症状或影响肝功能;(2)进行性增大;(3)不能除外恶性肿瘤或肝腺瘤。

Focal nodular hyperplasia of liver：a retrospective analysis in 58 patients

Objective To investigate the clinical diagnosis and surgical indication of focal nodu- lar hyperplasia(FNH)of the liver.Methods The clinical,pathological and imaging data of 58 pa- tients with liver FNH treated in our hospital from 1996 to 2002 were retrospectively analyzed.Results The liver FNH was most often seen in males(female-to-male ratio:1.0/3.5)and those patients with- out history of oral administration of contraceptive steroid(54/58).Most of the patients were asympto- matic and the serum test of liver was normal.The characteristic features were seen in helical multipha- sic liver CT(13/18)and MRI(11/16).The operative mortality was 0.There was no death during the follow-up.Conclusion The liver FNH has it special characteristics on the multiphasic helical CT scanning and MRI.The patients can be periodically followed up with imaging tests.Surgical resection should only be performed in symptomatic patients in case of mass growth or of uncertain diagnosis to exclude a malignant process or liver cell adenoma.
