中药联合早期肠内营养在治疗重症急性胰腺炎中的疗效观察

目的观察中药大黄、玄明粉联合早期肠内营养治疗重症急性胰腺炎（SAP）的疗效。方法将68例SAP患者随机分为全胃肠外营养（TPN）组、大黄联合早期肠内营养组及大黄、玄明粉联合早期肠内营养组。观察三组患者住院时间、住院费用、肠道功能恢复时间、血CRP、APACHEⅡ评分等，并进行比较。结果大黄、玄明粉组在住院时间、住院费用、肠道功能恢复时间上均低于其他两组（与TPN组比，P〈0．01；与单纯大黄组比，P〈0．05）。大黄、玄明粉联合早期肠内营养组住院1周A—PACHEⅡ评分、血CRP明显下降（与TPN组比，P〈0．01）。结论大黄、玄明粉联合早期肠内营养在治疗SAP中，能更有效地促进肠道功能恢复、降低住院费用、缩短住院时间，值得临床推广。

Therapeutic Effect of Combining theTraditional Chinese Medicine with Early Enteral Nutrition on Treating Severe Acute Pancreatitis

Objective To investigate the effect of combining the traditional Chinese medicine （TCM）, which are Radix et Rhizoma Rhei and Natrii Sulfas Exsiccatus, with early enteral nutrition on treating severe acute pancreatitis（SAP）. Methods All of 68 cases of patients with SAP were randomly divided into three groups. Group 1 was total parenteral nutrition （TPN）, Group 2 was Radix et Rhizoma Rhei combining with early enteral nutrition, Group 3 was Radix et Rhizoma Rhei and Natrii Sulfas Exsiccatus combining with early enteral nutrition. Length of stay, cost of hospitalization and times of functional recovery of intestinal tract were observed in three groups, and the results obtained were compared. Results For Length of stay, cost of hospitalization and times of functional recovery of intestinal tract, there were significant differences between 3 group and the two other group （P〈0.01 vs. 1 group, P〈0.05 vs. 2 group）. Score of APACHE Ⅱ and CRP of blood were significantly reduced in 2 group and 3 group compared to 1 group after a week in the hospital （P〈0.01）. Conclusion This method of combining with TCM is better in treating SAP for improving functional recovery of intestinal tract, lowering cost of hospitalization, and shortening length of stay, thus worth being popularized in clinical.
