硬膜外自控镇痛在胎盘植入中的镇痛效果及对应激反应的影响

目的探讨硬膜外自控镇痛在胎盘植入中的镇痛效果及对应激反应的影响研究。方法选取2017年6月-2019年6月天津医科大学第一中心医院收治的接受全子宫切除术治疗的胎盘置入患者102例,按照数字表法将其随机分为两组,对照组术后应用静脉镇痛,研究组术后应用硬膜外自控镇痛,分析两组患者镇痛效果以及对患者应激反应得的影响。结果研究组术后疼痛情况与对照组无明显差异(P>0.05);研究组镇痛优良率与对照组无明显差异(P>0.05);研究组皮质醇、β-内啡肽低于对照组(P<0.05);研究组不良反应低于对照组(P<0.05)。结论对于接受全子宫切除术治疗的凶险性胎盘前置患者采用静脉自控镇痛与硬膜外自控镇痛具有相同的镇痛效果,但是相对于静脉镇痛,硬膜外镇痛可以明显降低患者术后应激反应,具有更高的安全性。

Effect of epidural analgesia in placental implantation and its effect on stress response

Objective To investigate the analgesic effect of epidural controlled analgesia in placenta implantation and its effect on stress response. Methods select in June 2017-June 2019 of our hospital treated with total hysterectomy in 102 cases of patients with placenta,in accordance with the digital meter method will be randomly divided into two groups,control group postoperative intravenous analgesia,the team postoperative epidural controlled analgesia,analgesic effect and the analysis of two groups of patients the effect of stress reaction. Results there was no significant difference in postoperative pain between the study group and the control group( P>0. 05). There was no significant difference between the study group and the control group( P> 0. 05). Cortisol and beta-endorphins in the study group were lower than those in the control group( P<0. 05). The adverse reactions in the study group were lower than those in the control group( P<0. 05). Conclusion for the whole hysterectomy to treat the dangerous sex of patients with placenta previa,intravenous self-control analgesia and the analgesic effect of epidural controlled analgesia with the same,but compared with intravenous analgesia,patients with epidural analgesia can significantly reduce the postoperative stress reaction,with higher security,can be further popularization and application in clinical.
