阿司匹林联合双嘧达莫及丙种球蛋白治疗川崎病的疗效及对患儿凝血功能和免疫功能的影响

摘    要:目的评价阿司匹林联合双嘧达莫及丙种球蛋白治疗川崎病的疗效及对患儿凝血功能和免疫功能的影响。方法将2019年3月至2021年2月河南科技大学第一附属医院诊治的川崎病患儿82例,按抽签法分为对照组和观察组各41例,对照组使用阿司匹林+丙种球蛋白治疗,观察组使用阿司匹林+丙种球蛋白+双嘧达莫治疗。比较各组治疗前后相关凝血功能和免疫功能指标水平情况,以及总体疗效。结果两组治疗前凝血酶原时间(PT)、凝血酶时间(TT)、活化部分凝血活酶时间(APTT)和纤维蛋白原(Fbg)等凝血功能指标水平,以及免疫球蛋白A (IgA)、免疫球蛋白G (IgG)和免疫球蛋白M (IgM)等免疫功能指标水平比较,差异无统计学意义(P>0.05);治疗后各组PT、TT和APTT水平比较,差异无统计学意义(P>0.05),但观察组Fbg水平较对照组低(P<0.05);治疗后观察组IgA、IgG和IgM水平均较对照组高(P<0.05);总体疗效观察组较对照组高(P<0.05)。结论在川崎病的治疗中使用阿司匹林联合双嘧达莫及丙种球蛋白疗效确切,有利于改善患儿凝血功能障碍,增加其机体免疫能力,提升总体疗效。

Abstract:【Objective】 To evaluate the efficacy of aspirin combined with dipyridamole and gamma globulin in the treatment of Kawasaki disease and its effect on blood coagulation and immune function in children. 【Methods】 Eighty-two children with Kawasaki disease treated from March 2019 to February 2021 were divided into control group and observation group by lot. The control group was treated with aspirin+gamma globulin, and the observation group was treated with aspirin+gamma globulin+dipyridamole. The levels of relevant coagulation function and immune function indexes and the overall curative effect were compared before and after treatment. 【Results】 The levels of prothrombin time(PT), thrombin time(TT), activated partial thromboplastin time(APTT) and fibrinogen(Fbg) before treatment and the levels of immunoglobulin A(IgA), immunoglobulin G(IgG) and immunoglobulin M(IgM) in each group were compared(P>0.05). After treatment the levels of PT, TT and APTT between the two groups were not statistically different(P>0.05), but the level of Fbg in the observation group was lower than that in the control group(P<0.05);the levels of IgA, IgG and IgM in the observation group were higher than those in the control group(P<0.05);the overall curative effect was 95.12% in the observation group, which was higher than 75.61% in the control group(P<0.05).【Conclusion】 Aspirin combined with dipyridamole and gamma globulin is effective in the treatment of Kawasaki disease, which is conducive to improve children’s coagulation dysfunction, increase their immune ability and improve the overall curative effect.
