65岁以上老年女性乳腺癌206例治疗分析

目的探讨老年女性乳腺癌的临床特点和治疗选择。方法回顾分析赤峰市医院乳腺科1998年1月至2012年12月收治的206例老年女性乳腺癌患者的临床资料和随访结果。分析老年女性乳腺癌临床特点、治疗选择和影响预后的因素。结果全组206例老年乳腺癌占同期收治的1 212例乳腺癌患者的17%。其中78.16%（161例）的患者以乳腺肿物就诊。主要病理类型为浸润性导管癌（164例,79.6%）,雌和（或）孕激素受体阳性率为75.7%（156例）,Cerb-2：（＋＋＋）占16.0%（33例）。全组患者5年、10年生存率分别为78.0%和43.6%。TNM分期,免疫组化结果,脉管癌栓,手术选择和辅助综合治疗是影响预后的因素。结论老年女性乳腺癌手术和内分泌治疗是最重要和有效的首选治疗方式。影响预后的主要因素是TNM分期、免疫组化结果、脉管癌栓和治疗方式的选择。

Study on Diagnosis and Treatment of Elderly Breast Cancer Patients Over 65 Years Old,a Report of 206 Cases

Objective To analyze the clinical characteristics and therapeutics of elderly patients with breast carcinomas.Methods A total of 206 patients（≥65 years old）admitted into Chifeng City Hospital from Jan1998 to Dec 2012 were included for this retrospective study.Their clinical characteristics.treatment options and prognostic factors were analyzed.Results They accounted for17%（206/1 212）of breast cancer patients treated during the same period.The predominant complaint was palpable mass（n=161,78.16%）.The major pathologic type was invasive ductal carcinoma（n=164,79.6%）.The positive rate of estrogen receptor（ER）and（or）progesterone receptor（PR）was 75.7%（n=156）,and16.0%（n=33）cases were Cerb-2（＋＋＋）.The 5,10-year overall survival were 78.0%and43.6%respectively.TNM stage,Immunohistochemical results,vascular thrombosis,operative regimen and adjuvant therapy were prognostic factors.Conclusions Surgical operation and endocrine therapy are the first choice remedies for elderly patients.And the major prognostic factors are TNM stage,Immunohistochemical results,vascular thrombosis and treatment regimens.
