64层螺旋CT前瞻性心电门控CT冠脉成像的临床应用研究

摘    要:目的评价64层螺旋CT前瞻性心电门控技术冠脉成像应用于低心率患者的临床价值。方法本研究连续选取拟行CTCA检查的病人100例(心率〈65 bpm,HRv〈10 bpm),所选病人随机分别进行前瞻性心电门控(52例)及回顾性心电门控(48例)CTCA,依据病人体质量指数选择管电流(400~800mA)、管电压为120 kV。前门控组采集期像设定为75%,padding100 ms,后门控采用ECG管电流调制技术,35%~85%满电流输出。比较前门控组与后门控组的辐射剂量及图像质量。P〈0.05认为差异具有统计学意义。结果前门控组与后门控组图像质量差异无统计学意义(Z=-1.19,P=0.16)。前门控组CTCA辐射剂量平均(3.99±1.16)mSv,后门控组CTCA辐射剂量平均(15.49±3.96)mSv,差异有统计学意义(t=-16.12,P=0.00),使用前门控降低了74.24%的辐射剂量。结论前门控应用于低心率且心率相对稳定的患者中可以达到与后门控相仿的图像质量,并且可以满足诊断要求,而辐射剂量大幅减低。

Abstract:Objective To evaluate the 64-slice CT coronary Prospective ECG-gated imaging technology used in clinical value in patients with a low heart rate.Methods This study selected patients undergoing continuous CTCA examination 100 cases(heart rate〈65 bpm,HRv〈10 bpm),randomly selected patients were prospectively ECG-gated(52 cases)and retrospective ECG gating(48 cases)CTCA,based on patient body mass index options tube current(400~800 mA),tube voltage of 120 kV.Like the front door of the control group collection is set to 75%,padding 100 ms,backdoor controlled tube current modulation technique using ECG,35%to 85%percent over current output.Radiation dose and image quality is the front door and back door control group control group.considered statistically significant(P〈0.05).Results Front door and back door control control group was not statistically significant group of image quality(Z=-1.19,P=0.16).Qianmen CTCA radiation dose control group average(3.99±1.16)radiation dose mSv,backdoor control group CTCA average(15.49±3.96)mSv,the difference was statistically significant(t=-16.12,P=0.00),use front control reduces 74.24%of the radiation dose.Conclusion Door control for low heart rates and the heart rate is relatively stable patients rear door control can be achieved with similar image quality,and to meet the requirements of the diagnosis,and to substantially reduce the radiation dose.
