摘    要:目的 研究慢性阻塞性肺疾病合并慢性呼吸衰竭患者血乳酸联合BNP检测对患者预后的影响.方法 随机选择2014年1月至2016年1月我院收治的79例慢性阻塞性肺疾病合并慢性呼吸衰竭患者,患者按治疗效果分为A组(有效)和B组(无效).观测记录患者治疗前后的pH值,动脉二氧化碳分压(PaCO2)、动脉血氧分压(PaO2)、血乳酸及血浆脑钠肽(BNP)的水平.结果 B组起始的血乳酸和BNP明显高于A组,A组中血乳酸和BNP在24 h内降为正常,而B组24 h血乳糖和BNP水平持续升高,且持续时间明显长于A组,差异具有统计学意义(P<0.05).B组的pH值、PaO2明显低于A组pH值、PaO2,PaCO2明显高于A组PaCO2,差异有统计学意义(P<0.05).结论 采用血乳酸联合BNP检测评价COPD合并慢性呼吸衰竭患者,及时反映患者体内血乳酸、BNP水平,准确及时评价患者的治疗效果及预后.

Abstract:Objective To study the prognostic value of blood lactate combined with BNP in patients with chronic obstructive pulmonary disease and chronic respiratory failure.Methods 79 cases of chronic obstructive pulmonary disease complicated with chronic respiratory failure patients were randomly selected from January 2014 to January 2016 in our hospital.According to the results of treatment,patients were divided into the group A(valid)and the group B(invalid).The pH value,arterial partial pressure of carbon dioxide(PaCO2), arterial oxygen pressure(PaO2),blood lactate and plasma brain natriuretic peptide(BNP)level were recorded before and after treatment.Results The initial group B blood lactate and BNP were significantly higher than the group A,then the blood lactic acid and BNP of the group A reduced to normal at 24h,the group B 24 h lactose and BNP concentration increased,and the duration was significantly higher than that in the cured group,the difference was statistically significant(P<0.05).The group B of pH,PaO2was significantly lower than the group A pH,PaO2,PaCO2 was significantly higher than that in the group A PaCO2,the difference was statistically significant(P <0.05).Conclusion Blood lactic acid combined with BNP was used to evaluate COPD in patients with chronic respiratory failure,and to reflect the blood lactate and BNP levels dynamically,and to evaluate the treatment effect and prognosis of the patients accurately and timely.
