| 摘 要： | 目的:评价纤支镜检查对咯血患者病因诊断的价值。方法:对562例咯血患者采用纤支镜(FB)检查,其中38例可疑病例行重复纤支镜检查(RFB)。分别采用活检、刷检、针吸活检(TBNA)、经支气管肺活检(TBLB)、支气管肺泡灌洗(BAL)或多种方法联用,根据镜下病理学诊断分析病因。结果:首次FB检查确诊501例(89.15%),RFB检查38例中确诊36例(94.74%),联合其他方法后肺癌总检出率77.58%(436/562),其中由纤支镜确诊的肺癌411例(94.27%)。结论:纤支镜检查是确诊咯血病因的重要手段,重复纤支镜检查可提高诊断的阳性率。

| Abstract: | Objective:To evaluate the value of fiberbronchoscopy(FB) in diagnosis of causes of hemoptysis.Methods:562 cases of hemoptysis performed FB including 38 repeated FB.The biopsy,brush biopsy,trans-bronchoscopic needle aspiration(TBNA),transbronchial-lung biopsy(TBLB),bronchial alveolar lavage were used to find out the pathological causes of hemoptysis.Results:The causes of hemoptysis was idintified by FB in 501 of 562 cases(89.15%),and by RFB in 36 of 38 cases(94.74%).The tatal rate of diagnosed lung cancer was 77.58% by combined techniques including 411 cases by RFB(94.27%).Conclusion:RFB is the main technique for diagnosing the causes of hemoptysis,repeated RFB may improve the positive diagnostic rate. |
