200例脑梗死患者颈动脉超声检查结果分析

目的探讨动脉粥样硬化性脑梗死患者颈部动脉血管形态的变化,为脑梗死二级预防提供治疗依据。方法应用PHLIPS-5000彩色多普勒超声仪对200例经CT、MRI证实的脑梗死患者进行颈动脉超声检查,脑梗死按脑CT或MRI结果分为皮质组、皮质下组、皮质与皮质下混合组和椎基底动脉供血组,观察血管形态、内膜情况,有无斑块及斑块大小、形态、性质及血流动力学情况,管腔是否狭窄及狭窄程度。结果脑梗死患者大部分存在颈动脉斑块及不同程度颈动脉狭窄。170例检出颈动脉斑块的患者中,以不规则、不均质性回声斑块多见(P<0.001)。结论颈动脉粥样硬化(CAS)所致颈动脉血流动力学障碍及斑块性质与各种类型脑梗死的发生密切相关。超声检查能早期发现CAS,有利于脑梗死的治疗。

Result analysis of 200 patients with cerebral infarction examined with carotid artery ultrasound

Objective It is to discuss the blood vessel shape change of carotid artery in cerebral infarction patients with atherosclerosis in order to offer treatment basis for secondary prevention of cerebral infarction. Methods 200 patients with cerebral infarction who were confirmed with CT or MRI were examined with PHLIPS-5000 color Doppler ultrasound. The patients were divided into cortex group, subcortex group, cortex and subcortex complex group and vertebrobasilar blood supply group according to brain CT or MRI results. The blood vessel shape, intima circumstances, plaque or not, size, shape and character of plaque, hemodynamics, lumens stenosis or not and stenosis degree were observed. Results There were carotid artery plaque and different degree carotid artery stenosis in mostly cerebral infarction patients. The anomalism inhomogeneity echo plaque was the most in 70 patients with carotid artery plaque ( P <0.01). Conclusion The carotid artery hemodynamics disturbance and plaque character which are induced by carotid atherosclerosis are close correlated with the occurrence of various kinds cerebral infarction. Ultrasonic inspection can discover carotid atherosclerosis in earlier period, and it is useful for cerebral infarction therapy.
