电视胸腔镜手术在胸部疾病诊治中的应用

目的：总结电视胸腔镜外科（VATS）在胸外科疾病诊治的经验。方法：对165例肺癌及弥漫性胸膜间皮瘤合并的胸腔积液、乳糜胸、气胸并肺大泡、肺巨大囊肿或者肺大泡、肺不明结节或单个转移瘤、食管癌、弥漫性肺病变、外伤性和自发性血气胸、纵隔肿瘤等病例采用VATS辅助小切口操作法治疗。结果：手术创伤小，术后恢复快，诊断率高，疗效好，但仍有一些并发症。辅助小切口使手术安全性增高。结论：VATS的适应证除早期应用于简单的病种外，现在复杂手术在不断增多，但对术者和麻醉要求较高。

Diagnosis and Treatment of Thoracic Disease with Video-assisted Thoracoscopic Surgery

Objective\ To summarize experiences of application of video\|assisted thoracoscopic surgery(VATS) in diagnosis and treatment of thoracic surgery through analysis of 165 cases application of VATS.\ Methods\ VATS was applied to the cases of lung cancer, diffuse pleural mesothelioma combined with pleural fluid, chylothorx, pneumothorax combined with bullous lesions of the pulmonary, pulmonary gigantic cyst or bullous lesions of the pulmonary, unknown tubercula of or single metastasis tumor, esophageal carcinoma, diffuse pulmonary lesions, traumatic or spontaneous bloody\|pneumothorax, mediastinal tumors and so on.\ VATS assisted minithoractomy was adopted in some cases.\ Results\ VATS had advantages of slighter operative wound, faster postoperative recovery, less complications, higher diagnosis rate, better curative effect, but has some complications as well.\ Assisted minithoractomy operation raise safety.\ Conclusion\ The application range of VATS is extending.\ Besides some simple illness of early stages, the complexity of operation is increasing.\ So it is sure that VATS will be developed to a successful procedure in the diagnosis and treatment of thoracic diseases.\ But VATS has the limitations of high cost and high demands for surgeons and anesthesia.
