脐尿管囊肿超声诊断分析

目的探讨脐尿管囊肿的超声声像图表现,总结规律,提高诊断符合率。方法回顾性分析、观察12例脐尿管囊肿声像图表现,并与手术结果对比。结果 12例患者均经手术证实为脐尿管囊肿。单纯囊肿8例,超声诊断符合率75%;其中单发单纯囊肿6例,诊断符合率100%;2例为在脐尿管走行区有2个单纯囊肿,术前未做出直接诊断。声像图为于脐部至膀胱尖之间腹壁内显示圆形无回声,边界清晰,形态规则,壁薄且光滑,内部透声好,无血流信号,不与膀胱相通。脐尿管囊肿伴感染4例,超声诊断符合率50%。其中2例表现为无回声囊壁增厚内部呈细密点状低回声,透声差,囊壁增厚,不光滑;2例表现为下腹壁内形态不规则的低回声,位于膀胱尖部,与膀胱壁分界不清,膀胱顶部局部壁增厚。结论典型的脐尿管囊肿超声声像图有较高的诊断符合率,对于合并感染的脐尿管囊肿在结合临床症状及体征之后可以提高诊断符合率。

Research for urachal cyst by ultrasound: Clinical analysis

Objective To investigate the ultrasonographic characteristics of the urachal cyst in order to find out the rule and to sum up the experience and lessons for improving the coincidence rate of diagnosis.Methods Ultrasonographic characteristics of 12 patients were observed and compared with their operation results.Results The 12 patients were confirmed as urachal cyst by surgery;8 cases were pure cysts,coincidence rate of ultrasound diagnostic was 75%.Pure cysts was in 6 cases,the coincidence rate was 100%;2 cases had 2 pure cysts in urachal regiona,which did not made sure diagnosis before surgery;ultrasonographic displaied no echoes performance between the umbilicus and apex of the bladder.Boundary of cyst was clear,morphological rules,bursa wall thin and smooth,internal perforated was well,had no blood flow signals,and had no interlinked with bladder;4 patients were with infection,the coincidence rate of vultrasound diagnostic accuracy was 50%.And 2 cases without echo displays from internal was close plaque-forming hypoechoic,perforated poor,bursa wall thickening,not smooth.Under inside abdominal wall of 2 cases irregular hypoechoic showed,which was located at bladder tip of bladder wall,and had no clear boundary.Conclusion The diagnostic accuracy of typical urachal cyst is higher,and diagnostic accuracy of amalgamative infection of urachal cyst by combination with clinical symptoms and signs could be improved.
