高血压急症患者静脉应用地尔硫革和硝酸甘油疗效观察

目的:比较静脉注射地尔硫革和硝酸甘油治疗高血压急症患者的疗效.方法:37例舒张压≥110mmHg和(或)收缩压≥180 mmHg的高血压急症患者随机分为地尔硫(廾桌)组(n=18)和硝酸甘油组(n=19),分别应用地尔硫(廾桌)和硝酸甘油50mg和20 mg静脉注射,观察用药后5、10、15、30和60 min时血压和心率.结果:和用药前比较,两种药物均能显著降低血压(P<0.01),但硝酸甘油对降低舒张压起效稍慢,而对收缩压降压效果更明显;两种药物的减慢心率作用无显著差异;治疗60 min后,两组有效率分别为94.1%和100%(P>0.05).结论:静脉注射地尔硫革和硝酸甘油治疗高血压急症均安全有效.

Effect Observation of Intravenous Diltiazem and Nitroglycerin in Patients with Hypertension Emergency

Objective： To compare the effect of intravenous （Ⅳ） dihiazem and nitroglycerin in patients with hypertension emergency （HE）. Methods： 37 patients with diastolic blood pressure （DBP）≥ 110 mmHg and （or）systolic blood pressure（SBP）≥ 180 mmHg were divided into dihiazem group （50 mg, Ⅳ ,n=18） or nitroglycerin group （20mg, Ⅳ,n=19）. Blood pressures and heart rate were measured at 5, 10, 15, 30 and 60 minutes. Results： A significant decrease in the blood pressure of HE patients were observed in both treatment groups （P〈0.01）. However, the decrease in DBP was slower in the nitroglycerin group than that in the diltiazem group. While SBP decreased was significantly higher in the nitroglycerin group. Meanwhile, a slight decline in heart rate was observed in both groups. The effective rate at 60 minutes treated after was 94.1% and 100% in dihiazem group and nitroglycerin group respectively （P〉0,05）. Conclusion： Both diltiazem and nitroglycerin are safe and effective in controlling the blood pressure of HE.
