颅咽管瘤的MRI诊断

目的分析颅咽管瘤的MR I表现,评价MR I对颅咽管瘤的诊断及鉴别诊断价值。方法回顾性分析24例经手术病理证实的颅咽管瘤的磁共振成像(MR I)表现,其中13例有CT对照。结果24例颅咽管瘤发生于鞍上12例,鞍内3例,鞍内鞍上8例,鼻咽部1例。囊性5例,囊实性16例,实性3例。囊性部分T1W I信号表现多样,可呈低、中、高信号,T2W I均为高信号,实性部分多表现为等T1长T2信号,Gd-DTPA增强扫描后,囊性部分的囊壁呈弧形或环形强化,而实性部分不均匀强化。结论MR I是诊断颅咽管瘤的有效方法,且具有很好的定位及定性诊断价值。

MRI diagnosis of craniopharyngioma

Objective To evaluate the MRI diagnostic and differentiate values of craniopharyngioma and analyze MRI manifestations of craniopharyngioma.Methods MRI manifestations of 24 cases of craniopharyngioma confirmed by operation and histopathology were analyzed retrospectively and compared with CT in 13 cases.Results Twelve tumors were located in suprasellar,3 in sellar,8 both in sellar and suprasellar and 1 in nasopharynx.Five of the tumors were cystic,16 were solid-cystic and 3 were solid in nature.The signals of cystic part on T_1WI appeard various.It could be hypointensity,isointensity or hyperintensity.The signals of cystic part on T_2WI were hyperintensity.Signal of solid part was often equal T_1 and long T_2.The appearances of post-contrast images were typical.The wall of the cystic part was curvilinear or circular enhancement,and solid part was nonhomogeneous enhancement.Conclusion MRI is an effective method to diagnose craniopharyngioma,and it has great value in level diagnosis and qualitative analysis.
