治未病模式对社区老年高血压健康管理

对老年高血压在社区高血压慢病自我管理基础上,进行"治未病"预防保健综合干预早期防治的社区实践,初步建立并验证该特色服务模式的有效性和适用性,形成中医药综合干预方案;建立基于社区卫生服务中心,以三级综合性中医院为核心的高血压病早防、早治模式,为高血压中医治未病服务模式干预的社区综合防治体系提供可行依据,使高血压病防治重点从专科诊疗向社区预防转移。社区卫生简便低廉的"六位一体"服务功能以及中医治未病的预防保健优势,对老年高血压病的防治是有效、可行的。
