辨证分型、药浴与针刺联合西药治疗糖尿病周围神经病变随机平行对照研究

目的]观察辨证分型、药浴与针刺联合西药治疗糖尿病周围神经病变疗效。方法]使用随机平行对照方法,将96例住院患者按就诊顺序号法简单随机分为两组。口服降糖药、注射胰岛素。对照组48例甲钴胺,50mg/次,3次/d。治疗组48例辨证分型:脾虚气弱,理气通络,健脾除湿(党参18g,茯苓、薏苡仁、苍术、木瓜各6g,威灵仙、半夏、白芥子各3g);血疲阴虚,滋补肝肾、通络养血(桑葚子9g,何首乌、熟地黄、五加皮各6g,生地黄、怀牛膝、鸡血藤、白芍、海风藤各3g);1剂/d,水煎300m L,1次/d;药浴(细辛、艾叶各30g,伸筋草、透骨草、桂枝各20g,丹参15g,鸡血藤12g,冰片9g,乳香、红花各6g),水煎3000m L,温度维持在40℃,置于足浴桶浸泡患肢,早晚各浸泡一次。针刺:足三里、气海、血海,10min/次,1次/d。连续治疗7d为1疗程。观测临床症状、肢体麻木、萎缩、不良反应。连续治疗2疗程,判定疗效。结果]治疗组痊愈30例,有效17例,无效1例,总有效率97.92%。对照组痊愈18例,有效22例,无效8例,总有效率83.33%。治疗组疗效优于对照组(P0.05)。结论]辨证分型、药浴与针刺联合西药治疗糖尿病周围神经病变效果显著,值得推广。

Treatment group, 48 cases Syndromes： spleen qi is weak, cure qi meridians, spleen dehumidification （ Dangshen 18g, Fuling,Yiyiren,Cangzhu,Mugua each 6g, Weilingxian,Banxia,Baijiezi 3g ） ; blood fatigue deficiency, treatment with nourishing liver and kidney meridians nourishing （ Sangshenzi 9g, Heshouwu,Shudihuang,Wujiapi each6g, Shengdihuang,Huainiuxi,Jixueteng,Baishao,Mugua3g ） ; l/d, decoction 300mL, 1 times/d. Medicated bath to improve symptoms in patients with lesions in the extremities （ Xixin, Aiye 30g,Shenjincao,Tougucao,Guizhi 20g, Danshen15g, Jixuetcng12g,Bingpian 9g,Ruxiang,Honghua6g ） , decoction 3000mL, the temperature was maintained at 40℃, placed limb foot tub soak, soak time each morning and evening. Acupuncture Zusanli, sea air, a sea of blood, 10min/times, 1 times/d. 7d is a continuous course of treatment. Observing clinical symptoms of adverse reactions. 2 courses of continuous treatment, to determine efficacy. Results ] The cure 30 cases, effective in 17 cases, one case, the total efficiency of 97.92%. The control group healed in 18 cases, effective in 22 cases, 8 cases, the total effective rate was 83.33%. Treatment group than the control group （ P〈0.05 ） . Conclusion] Syndrome type joint medicated bath and peripheral neuropathy effects of acupuncture treatment of diabetes significantly, worthy of promotion.
