中医药防护糖尿病肾病肾脏内皮细胞损伤研究概况

肾脏内皮细胞损伤是糖尿病肾病(Diabetic Nephropathy,DN)早期特征性改变之一,中医药治疗DN良好疗效已得到证实,研究多集中在肾小球系膜细胞及上皮细胞,近年针对肾脏内皮细胞功能研究取得可喜进展。多数学者认为DN属本虚标实,气阴两虚为本,瘀血阻络为标,贯穿始终,治疗原则益气养阴化瘀通络,实质就是控制糖尿病(降糖、降压、降脂、抗凝)、阻止肾脏纤维化、保护肾功能、阻止蛋白尿。实验和临床均显示中医药能较好保护肾脏血管内皮、降低内皮素病理性升高、抑制NO减少、恢复内皮相关细胞因子调节等。多数研究停留在个别环节,内在作用机理研究较少见,更没能体现出多途径、多靶点的中医药复方优势;缺乏针对性靶点,内在机制研究方面不透彻,没有阐明药物作用系列机制,指标检测也缺乏连贯性,未来结合基因组学与蛋白组学技术,研究中医药保护肾脏内皮细胞和使再生内皮细胞尽快恢复正常表型作用机制将是必然趋势,以期在新药开发,防治疾病上有所突破。

The Research Work of Protection of Traditional Chinese Medicine Diabetic Nephropathy Renal Endothelial Cell Injury

The good efficacy of traditional Chinese madicine in the treatment of DN has been confirmed in a large number of experimental and clinical.and research focus in the glomerular mesangial cell and epithelial cells.Studies on renal endothelial function made gratifying progress in recent years .Most scholars believe that the DN is the virtual to real. Qi and Yin two empty as this and blood stasis of collaterals as standard,which though of the DN.The therapeutic principles is supplement qi and nourisgeyin and dredg meridians,which the essence is control diabetes so as to prevent kidney fibrosis,protect kidney function,prevent proteinuria. Experimental and clinical results show that traditional Chinese medicine has good protetion function for kidney impaired endthelial,which can reduce endthelin pathological rise,NO less,restore the endthelial cells associated factor adjustment disorder,etc.But most of the research are staying in individual link . There is little other intrinsic mechanism research.It cannot reflect ways and targets of the traditional Chinese medicine compound advantage.It is short of specific targets and is not thorough in internal mechanism research,and it do not illuminate series mechanism of drug action.The index testing alse are lack of consistency.So the mechanism of researing traditional Chinese medicine protect endothelial cells in the kidney and regenerate endothelial cells back to normal phenotypes as soon as possible is the inevitable trend in combination with genomics and proteomics technology in the future.It will be have a breakthrough in the development of new drugs and in the prevention and treatment of disease.
