独活寄生汤联合推拿正骨治疗腰椎间盘突出随机平行对照研究

[目的]观察独活寄生汤联合推拿正骨治疗腰椎间盘突出疗效。[方法]使用随机平行对照方法,将80例门诊及住院患者按抛硬币法简单随机分为两组。对照组40例独活寄生汤(独活、茯苓、桑寄生各20g,熟地黄30g,防风12g,川芎、人参各10g,当归、杜仲、秦艽、川牛膝、白芍各15g,肉桂、甘草各6g,细辛3g;气滞血瘀去肉桂、白芍、熟地黄、防风、秦艽,加三七、桃仁、红花、香附各10g,地龙12g;身重如裹、腰膝冷疼,风寒湿加制川乌、附片各6g,鸡血藤12g;气血不足、困倦乏力、素体虚弱,肾虚去茯苓、秦艽、防风、细辛,加黄芪、狗脊、淫羊藿、骨碎补各15g),1剂/d,水煎200mL,早晚口服;药渣涂抹患处。治疗组40例推拿正骨:取仰卧位,双指点于腰部,点按夹脊穴,持续2min;左手全掌按压腰椎突出处,抬单侧膝部,背侧过伸,身体成为1条直线;屈髋、屈膝,站于腹侧,左手按臀部,右肘平按臀髋后方,将躯干扭转,直至耐受,并轻轻晃动,待肌处放松,肘部向下外侧出力,听到咔嚓声,表示手法成功;独活寄生汤治疗同对照组。连续治疗14d为1疗程。观测临床症状、VAS疼痛评分、不良反应。连续治疗2疗程,判定疗效。[结果]治疗组显效28例,有效10例,无效2例,总有效率95.00%。对照组显效21例,有效9例,无效10例,总有效率75.00%。治疗组疗效优于对照组(P0.05)。VAS疼痛评分两组均有改善(P0.05),治疗组改善优于对照组(P0.05)。[结论]独活寄生汤联合推拿正骨治疗腰椎间盘突出效果显著,值得推广。
