独活寄生汤联合外用熏洗治疗寒湿腰痛60例临床观察

目的]观察独活寄生汤联合外用熏洗治疗寒湿腰痛疗效。方法]使用前瞻性设计方法,对60例门诊患者使用独活寄生汤(茯苓、独活、当归、秦艽、杜仲、川芎各10g,肉桂3~5g,人参、牛膝各10g,细辛3~6g,桑寄生15~20g,生地12~15g,白芍10~15g,甘草6g,干姜8g),痛感剧烈加制草乌、川乌、白花蛇,活血止痛、搜风通络;湿邪偏盛去生地,加苡仁、防己、苍术、九节茶,祛湿消肿止痛;寒邪偏盛加附子,温阳散寒;脊柱疼痛明显加地鳖虫、金毛狗脊、羌活;腰膝关节骨刺明显去杜仲,加补骨脂、骨碎补、没药、乳香、生龙骨。水煎300mL,1剂/d,早晚分服。外用熏洗方(红花、当归、制草乌、制川乌、川椒、露蜂房、麻黄、桂枝),煎煮取汁,30min/次,2次/d。连续治疗7d为1疗程。观测临床症状、不良反应。治疗1疗程,判定疗效。结果]痊愈56例,显效2例,有效1例,总有效率98.33%。结论]独活寄生汤联合外用熏洗治疗寒湿腰痛效果显著,值得推广。
