针刺麻醉联合关节松动术治疗膝关节功能障碍随机平行对照研究

[目的]观察针刺麻醉联合关节松动术治疗膝关节功能障碍疗效。[方法]使用随机平行对照方法,将98例住院患者按就诊顺序号简单随机分为两组。对照组49例关节松动训练,澳大利亚麦特兰德(Maitland)Ⅰ~Ⅳ级手法;Ⅰ~Ⅱ级用于关节疼痛;Ⅲ级用于关节疼痛伴僵硬,Ⅳ级用于治疗周围组织黏连,挛缩引起的关节大活动受限;30min/次,2次/d。治疗组49例在关节松动前加针刺麻醉,取血海、梁丘、风市、萁门、殷门、阳陵泉、委中、合谷、手三里,华佗牌针灸针40mm,针刺有酸麻涨困针感后,接华谊牌G6805-2A低频脉电子脉冲治疗仪,连续波200次/s,电流开至能耐受,10min调整一次共计30min;或下肢放松,疼痛刺激阈值提高后关节松动训练,15min靠墙下蹲训练,麻醉效果消失前做主动抗阻训练。连续治疗20d为1疗程。观测临床疗效、晨僵时间、关节功能(活动度)、不良反应。连续治疗3疗程,判定疗效。[结果]治疗组治愈39例,显效7例,有效3例,无效0例,总有效率100%。对照组治愈30例,显效8例,有效2例,无效9例,总有效率81.63%。治疗组疗效优于对照组(P0.05)。[结论]针刺麻醉联合关节松动术治疗膝关节功能障碍疗效显著,值得推广。

Continuous treatment of 20d 1 course of treatment. Observed the clinical effect, treatment time, adverse reaction. Continuous treatment 3 period of treatment, judge curative effect. [ Results J The treatment group cure 39 cases, 7 cases were markedly effective, effective in 3, 0 no effect, the total effective rate was 100%. 30 eases of control group cure, 8 cases were markedly effective, effective in 2 cases, 9 no effect, the total effective rate was 81.63%. The treatment group curative effect is better than that of control group （ P〈0.05 ） . [ Conclusion ] Joint joint manipulation therapy acupuncture anesthesia effect is remarkable, the knee joint dysfunction is worth promoting.
