摘    要:目的:心动周期中收缩期的时间分布是心脏收缩功能的重要指标。同一心动周期同步测量左右心室及室间隔收缩期间期非常困难,本文旨在探讨判断心脏收缩协调性的好方法。方法:用组织多普勒速度-时间曲线同时确定左右心室及室间隔等容收缩期、加速射血期和减速射血期的特征。31例健康志愿者,年龄(34.77±8.47)岁,最大42岁,最小25岁,其中男性17例,在心尖四腔图切面,在基底部和中部分别测量这些时间值。结果:左心室基底部等容收缩时间(63.33±13.73)ms,加速收缩时间(64.33±16.54)ms,减速收缩时间(246.67±22.49)ms,收缩达峰时间(127.67±19.78)ms。右心室基底部等容收缩时间(75.67±18.32)ms,加速收缩时间(98.33±27.30)ms,减速收缩时间(206.00±40.05)ms,收缩达峰时间(180.00±28.77)ms。结论:组织多普勒速度时间曲线是判断心室壁运动时间方便而准确的方法。

Abstract:objictive: Background there are studies on time intervals in cardiac cycle have shown that some time index (the Tei and PEP) were proposed as potentially useful predictors of global cardiac function. But it is difficulty to simultaneously measure contraction and relaxation time intervals of left and right ventricle. Methods: To validate the use of velocity Doppler tissue imaging that measures lime intervals of the myocardial performance and to define the characteristics of these time intervals in normal subjects, we obtained and compared the right and left ventricular time intervals in 31 volunteers. From apical four - chamber view, time intervals were measured within the basal and mid parts of the interventricular septum, and the left and right free ventricular wall in same heart cycle. Results: in contraction, contraction, rapid ejection period, peak systolic velocity period are longer in right ventricle than in left ventricle (p < 0. 01). The differences of ejection period and total systolic period in right ventricle than in left ventricle did not show statistically significant. In relaxation, isovolumic relaxation, diastasis are shorter in right ventricle than in left ventricle (p < 0.01). But rapid inflow and atrial systole are longer in right ventricle than in left (p < 0.01). The differences of total relaxation period between right ventricle and left ventricle did not show statistically significant. Conslusion: velocity Doppler tissue imaging is useful and accurate method to assessment time intervals in cardiac cycle.
