血管黏附分子在食管癌中的表达及其临床意义

摘    要:目的 :研究食管癌瘤体中血管黏附分子 (VCAM 1)及外周血血清中可溶性血管黏附分子 (sVCAM 1)的表达与食管癌的血管生长、转移方面的影响。方法 :对 4 1例食管癌、8例良性食管溃疡、10例正常人标本进行免疫组化染色和图像分析 ,同时用酶联免疫吸附试验对 2 5例正常人、30例食管癌术前和术后 1周外周血进行检测 ,分析瘤体及外周血清中VCAM 1的表达与食管癌病理参数的关系。结果 :VCAM 1不仅在食管癌血管内皮细胞上高表达 ,在食管癌细胞膜上也有表达 ,而且VCAM 1的表达强度、表达率与食管癌血管的生长和转移有关。食管癌组病人外周血清中sVCAM 1的浓度比正常对照组高 (P <0 .0 1) ,并且术后食管癌病人血清中sVCAM 1的浓度下降 (P <0 .0 1)。结论 :VCAM 1在食管癌的血管生成、癌细胞转移中可能起到重要作用 ,sVCAM 1有望成为食管癌的早期诊断及复发的指标。

Abstract:Objective:To investigate the expression of VCAM-1 in esophageal carcinoma and soluble VCAM-1 in peripheral blood and its role in the growth and metastasis of esophageal carcinoma. Methods: tissue specimens from 41 cases of esophageal carcinoma, 8 of benign esophageal ulcer and 10 normal controls were analyzed by immunohistochemistry and imaging techniques. Meanwhile, blood specimens from 25 normal controls, 30 cases of esophageal carcinoma collected pre-operatively and 1 week after operation were detected using ELISA. All the results were assessed for the relationship between VCAM-1 expression and clinical pathology of esophageal carcinoma. Results: high-level expression of VCAM was seen both in the vascular endothelium and the cell membrane of the tumors, and the rate and intensity of expression was associated with tumor growth and metastasis. The level of soluble VCAM-1 in serum of patients was higher than that in normals (P<0.01) and declined after operation (P<0.01).Conclusion: VCAM-1 may be playing an important role in the angiogenesis, metastasis of esophageal carcinoma, and soluble VCAM-1 may offer promise for the early diagnosis as a bio-marker.
