腹腔镜手术在妇科疾病中的应用

目的探讨腹腔镜手术在妇科疾病的应用。方法回顾性总结该科2001年10月￣2004年7月进行各种妇科腹腔镜手术共428例的资料。结果该组有10余种妇科疾病行腹腔镜手术治疗,近20种手术方式;中转开腹7例(1.67%);并发症发生率0.94%,无大血管损伤、空腔脏器损伤、电损伤和死亡等严重并发症。结论随着新技术、新器械的不断发展,腹腔镜手术的禁忌证逐渐减少。扎实的基本功、相对固定的手术团队、谨慎选择手术对象、及时中转开腹是预防和减少手术并发症的有效措施。腹腔镜手术对病人创伤小、术中出血少、术后恢复快、住院时间短、腹壁切口美观,而且诊断和治疗同步完成,因此,腹腔镜手术在妇科疾病的诊治中拥有广阔的应用前景。

Prevention of persistent ectopic pregnancy after TV laparoscopic surgery for tubal pregnancy

Objective] To observe the use of laparoscpic surgery in gynopathy. Methods] A retrospective summary for 428 cases of gynaecologic laparoscopy from Oct. 2001 to Jul.2004 was performed. Results] In the group, more than 10 kinds of gynopathy were treated by nearly 20 kinds of means through laparoscopy, 1.67 percent of which turned to be celiotomy, the incidence of complication is 0.94%. There was no serious incidence in the group, such as injury of main blood vessel and cavity organ, electric injury or death. Conclusion] The contraindication of laparoscopy become decreasing along with the developing of new technique and apparatus. Steady technique, standing team, carefully object seletion, and timely laparotomy might prevent the incidence of laparoscopy efficiently. Laparoscopy will be using widely for the benefit that small injury, less bleeding during operation, fast recover postopreration, short time in hospital and nice wound, and it also make dagnosis and therapy complate at the same time.
