多发性硬化患者智能、记忆及事件相关电位研究

摘    要:对2组共55例多发性硬化(MS)患者及其对照组采用韦氏成人智力量表、临床记忆量表和事件相关电位检查,结果表明:①MS病程中可出现不同程度的智能和记忆障碍,小于等于70~79分者的记忆商(MQ)、全量表智商(FIQ)、语言智商(VIQ)和操作智商(PIQ)分别为76%、32%、24%、70%。与对照组相比差异有非常显著性意义(P<0.005)。②MS脑或脑脊髓型MQ、FIQ较脊髓型成绩低下(P<0.005);MS活动期MQ、FIQ较非活动期成绩低下(P<0.001);临床表现为脊髓型54%有记忆障碍,12%有智能低下;MS智能障碍一般表现为皮质下痴呆的特征,在慢性进展型可表现为皮质兼皮质下痴呆的双重特征。③MS组N_2、P3波潜伏期延长,与对照组相比差异非常显著(P<0.001)。MS组P3异常率高于各分项智商异常率,差异非常显著(P<0.001)。

Abstract:patients with multiple sclerosis(MS) divided into two groups and theircontrol groups were tested with the Wechsler adult intelligence scale,clinical memory scaleand event-related potentials(ERP).The results indicated that:①Disturbances of intelli-gence and memory occur along the course of MS at various degrees. The memory quotient(MQ ),full intelligence quotient(FIQ ),verbal quotient(VIQ)and processing quotient(PIQ) of patients with the intelligence quotient(IQ)less than or equal to 70~79 scores are76%, 32%,24%and 70%respectively,with a difference markedly significant as comparedwith the control group,②The score of MQ and FIQ in patients with MS of cerebral type orcerebrospine type is lower than that of spine type(P<0.005);the score of patients with MSin active stage is less than that of MS in inactive stage(P<0.001);among patients withspine clinical manifestation , the occurence of memory disturbance is 54%and that of intelli-gence disturbance is 12%, suggesting that cerebral minilesions exist in part of spine type. Inpatients MS patinets usually is characterized by subcortical dementia. The intelligence dis-turbances of MS patients usually is characterized by subcortical dementia; in patients ofchronic active type , it may be characterized either by cortical dementia or subcortical demen-tia. ③The latent period of N2 ,P3 in MS is longer than the control group with a markedlysignificant difference (P<0.001).The occurence of abnormal P3 is higher than that of vari-ous abnormal IQ(P<0.001) suggesting that P3 may be an indice in the evaluation of cogni-tive disturbances.
