氟比洛芬酯对围术期根性神经痛的镇痛效果观察

目的为减轻围术期根性神经痛寻找较为理想的辅助药物。方法随机选择60例腰椎间盘摘除术的患者均分为3组,分别使用氟比洛芬酯（氟比洛芬酯组）、芬太尼（芬太尼组）治疗,另一组为未用药组,各组所用药物均在剥离椎间盘压迫的神经根前应用。观察各组患者的镇痛质量和不良反应的发生率。结果3组患者在切皮时的平均动脉压（MAP）、心率（HR）、血氧饱和度（SPO2）,与麻醉前比较差异无显著性（P〉0.05）;在剥离神经根时未用药组的MAP、HR明显增加,与术前及其他两组比较差异均有显著性（P〈0.05）。3组患者在切皮时的镇痛质量相似（P〉0.05）,在剥离神经根时氟比洛芬酯及芬太尼组具有相似的镇痛质量,差异无显著性（P〉0.05）。未用药的患者则痛感明显,与其他两组比较,差异有非常显著性（P〈0.01）。但芬太尼组的6例患者在俯卧位时出现明显的呼吸抑制,其它组病人则无此现象。结论氟比洛芬酯是减轻根性神经痛的较为理想的药物。

Analgesic Effect of Flurbiprofen Axetil on Lumbar Radicular Pain During Perioperative Period

Objective To find ideal adjunctive drugs to relieve lumbar radicular pain during perioperative period. Methods Sixty patients undergoing selective lumbar discectomy were randomly divided into three groups, flurbiprofen axetil group, fentanyl group, administrated before peeling off the intervertebral disc, and control group, and the analgesie effects and adverse reactions of three groups were observed. Results There were no significant differences at MAP, HR, SPO2 in all groups compared with preoperation （ P 〉 0.05 ） ; and there were obvious difference in the control group at MAP, HR compared with preoperation and the other groups when peeling off the intervertebral disc （ P 〈 0.05 ）. Similar analgesie effect was seen in all groups at the beginning of operations （ P 〉 0.05 ） ; there were no significant differences between the two groups which were administrated flurbiprofen axetil and fentanyl, when peeling off the intervertebral disc （ P 〉 0.05 ） , and there were obvious difference in the control group compared with the other two groups （ P 〈 0.01 ）. But six patients who were administrated fentanyl experienced respiratory depression. Conclusions Flurbiprofen axetil was an ideal adjunctive drug to relieve lumbar radicular pain during perioperative period.
