轻度胃肠炎伴发良性婴幼儿惊厥32例临床及病原学分析

目的：分析轻度胃肠炎伴发婴幼儿良性惊厥（BICE）的临床特点和预后。方法：对我院收治的32例BICE住院患儿进行临床观察和随访。结果：32例患儿发病年龄最小8个月,最大28个月。胃肠道症状出现2 d内发生惊厥28例（87.5%）,持续时间〈5 min;发作形式为全身强直-阵挛发作29例（90.6%）,局限性发作泛发为全面型发作3例（9.4%）。惊厥发作间期患儿脑电图、脑影像学、血生化、脑脊液检查均正常。大便轮状病毒阳性组和阴性组在BICE的发病年龄、发生时间、发作次数及丛集发作时间比较差异均无统计学意义（P＞0.05）。结论：BICE患儿消化系统表现较轻微,惊厥成簇发作,预后良好。临床上以对症治疗为主,应避免不必要的检查和过度的药物治疗。

Clinical and Etiological Analysis on Benign Infantile Convulsions Associated with Mild Gastroenteritis

Objective： To investigate the clinical features and prognosis of benign infantile convulsions associated with mild gastroenteritis（ BICE）. Methods： Thirty-two cases with gastroenteritis accompanied with convulsions were clinically observed and followed up for 6 ～ 12 months. Results： The ages of onset in the thirty-two cases ranged from 8 to 28 months old. Twenty-eight cases（ 87.5%） developed seizures in the first two days and persisted within five minutes. Twenty-nine cases（ 90. 6%） presented with generalized seizures. The electroencephalogram,brain imaging,blood biochemical profile and cerebrospinal fluid testing did not show abnormality in all cases. The differences of age of onset,time of occurrence,seizure frequency and cluster onset time between stool rotavirus-positive and negative groups with BICE were not statistically significant（ P〉0. 05）. Conclusions： The infants with BICE can be a good prognosis in the future,and antiepileptic therapy may not be necessary after seizure cessation.
