安心茶话屋在调查肺癌患者生命末期需求中的实践

摘    要:目的 开展安心茶话屋活动,了解肺癌患者的生命末期需求,提升患者面对死亡的认知,给予支持,协助其善用余生。方法 采取立意取样法对肺癌患者,使用美华慈心关怀联盟设计的安心卡,由经过安心卡使用培训的1名主持人和3名引导人合作完成安心卡茶话屋活动,并完成安心卡茶话屋愿望登记表。结果 (1)共开展7场安心卡茶话屋活动,服务患者38名,其中33名参加全程活动,5名中途离场。100%患者认为安心茶话屋活动是有意义,值得推广的。(2)共选中安心卡99张,其中肺癌患者生命末期的身体需求为30张(30%),人际需求30张(30%),心灵需求22张(22%),财务需求12张(12%),特别心愿5张(5%);(3)其中高频率出现安心卡的内容是,我不想成为家人的负担,我希望有信任的医生和亲切的护士,我希望家人能和睦相处。结论 安心茶话屋对于了解肺癌患者生命末期的需求,并且打开死亡的话题应用效果较好,能够让患者开始认同医疗照护事前指示。也为死亡教育的开展提供了一个简单易行的工具,指导舒缓疗护团队对恶性肿瘤患者提供身、心、社、灵全方位的关怀和照护。

Abstract:Objective To conduct Heart to Heart® Café to understand the end-of-life wishes of patients with lung cancer, enhance patients' cognition on death, and provide support for them to enjoy their remaining life. Methods Lung cancer patients were chosen with the method of purposive sampling. One host and three leading persons who were trained by Heart to Heart® card designed by the Chinese American Coalition for Compassionate Care held the Heart to Heart® Café. Patients completed Heart to Heart® card. Results Seven Heart to Heart® Café were launched including 38 patients, 33 of which participated in the whole activity and 5 left in the middle. All the participants believed such activities were meaningful and helpful and was worth to be popularized. A total of 99 Heart to Heart® card were collected. Among them, there were 30 cards of body needs of lung cancer patients at the end of life (30%), 30 of interpersonal needs (30%), 22 of spiritual needs (22%), 12 of financial needs (12%), and 5 of special wishes (5%). The most frequently seen wish was I do not want to become the family burden; I want a trusted doctor and a caring nurse, and I hope my family can get along well. Conclusion Heart to Heart® Café is conducive to understanding the concerns of lung cancer patients at the end of their life, to helping them face death positively. It is a easy but practical tool of death education for cancer patients and a good tool for soothing care to provide comprehensive care for cancer patients.
