老年慢性阻塞性肺病急性加重期痰培养病原菌及药敏分析

目的:总结老年慢性阻塞性肺病急性加重期(AECOPD)患者痰培养病原菌菌型分布及药敏的现状。方法:回顾了本院2 0 0 4年一年内72名老年AECOPD的患者痰标本细菌培养和药敏报告,并对所分离出来的革兰氏阴性杆菌(GNB)进行菌型分类。结果:老年AECOPD患者痰培养显示以革兰氏阴性杆菌为主,占83%。其中以非发酵菌为主,占4 2 %。其次为嗜血杆菌杆菌属和肠杆菌属。细菌依次为铜绿假单胞菌,流感嗜血杆菌,副流感嗜血杆菌,不动杆菌,克雷伯杆菌。药敏试验显示大多数革兰氏阴性杆菌对头胞三代,新喹诺酮类,氨基糖甙类,美洛培南等抗生素敏感。结论:革兰氏阴性杆菌是老年慢性阻塞性肺病急性加重期主要的致病菌。需合理根据老年人发病特点使用抗生素,才能更好的控制和缓解患者的症状。

Organism Distribution and Drug Sensitivity in the Period of Acute Exacerbation of the Elderly Patients with Chronic Obstructive Pulmonary

Obejective To summarize the character of pathogen and drug sensit ivity of phlegmy plant in the elderly patients with chronic obstructive pulmonar y. Methods To analyze the bacterial cultures of sputum and drug sens itivity of th e seventy-two elderly patients in the period of acute exacerbation of COPD.The s e patients came from Xiangya hospital in 2004.All the same time,the GNB were cla ssi fied. Results GNB is primary in the elderly patients which were in t he period of exacerbation of COPD,they were 83%,in which the NFC was 42%.The second is Hae mophilus influenzeand the third is Enterobacter.Most of the organism wa s Pseudo monas aeruginose, Haemophilus influenze,Acinetobacter, Klebsiella. The drug s ensiti vity showed most of the GNB were sensitive to ceftazidime,cefoperazone,vancomyci n,meropenem and so on. Conclusion GNB were the main organism in the elderly pati ents which were in the period of acute exacerbation of COPD.Antibiotic should be selected according to the character of the elderly patients, which can control and relief pains of the patients for the more.
