肌钙蛋白对病毒性脑炎合并心肌炎患儿的诊断价值

目的探讨病毒性脑炎时血清心肌肌钙蛋白I（cTnI）、肌酸激酶（CK）、肌酸激酶同工酶（CK-MB）对心肌损伤的诊断价值。方法对2005年8月～2008年12月64例病毒性脑炎患儿与对照组为同期入住非脑炎其他疾病抽搐患儿22例,分别于入院后第1、2天采集静脉血测定血清cTnI、CK、CK-MB,并进行分析。结果在研究中发现cTnI对病毒性脑炎合并心肌损伤有较高的敏感性,且比CK-MB及CK更具有特异性,是治疗和判断预后的指标之一。结论病毒性脑炎患儿心肌损伤与脑损伤密切相关,即脑损伤越重,心肌损伤也越重,外周血cTnI的检测能早期诊断心肌损伤,从而可以早期干预治疗,改善预后。

Diagnostic value of troponin on viral encephalitis and myocarditis in children

Objective To study viral encephalitis serum cardiac troponin I （cTnI）,Creatine kinase （CK）,Creatine kinase isoenzyme （CK--MB） for the diagnosis of myocardial injury. Methods August 2005 - December 2008 64 cases of viral encephalitis in children with the control group for the same period of stay of non--encephalitis in children with other diseases, treated 22 cases of convulsions respectively,after admission, paragraphs 1-- 2 days collecting blood Determination of cTnI,CK,CK-- MB, and for analysis. Results The importance of viral encephalitis in children with cardiac function monitoring and therapy, to improve the prognosis in children is of great significance, cTnl was found in the study of vi- ral encephalitis with myocardial damage had a higher sensitivity. And more than CK-- MB, CK was more specific, was to determine treatment and prognosis of one of the indicators. Conclusions In children with viral encephalitis and brain damage myocardial damage is closely related to, that is, the more serious brain damage, the more severe myocardial injury, cTnl detection in peripheral blood to the early diagnosis of myocardial damage, which early intervention therapy can improve the prognosis.
