七叶皂苷素对肢体缺血-再灌注脂质过氧化反应影响的临床观察

摘    要:目的观察七叶皂苷素对肢体缺血-再灌注脂质过氧化反应的临床效果。方法选择40例肢体手术需充气带加压肢体止血的患者,随机分为对照组(n=20)和治疗组(n=20)。治疗组患者术前10min静脉滴入七叶皂苷素注射液16mg,对照组患者给等量平衡液。肢体缺血前,再灌注后30、90、180min分别检测血丙二醛(MDA)、肌酸磷酸激酶(CPK)和超氧化物歧化酶(SOD)。结果肢体缺血-再灌注30、90、180min与缺血前比较,血MDA和CPK含量升高,SOD含量下降。治疗组缺血-再灌注同时间,血MDA含量明显低于对照组(P〈0.01);CPK值低于对照组(P〈0.05);SOD活性逐渐上升,180min高于对照组(P〈0.05)。结论七叶皂苷素有抗脂质过氧化作用,降低CPK值,提高SOD活性,能有效防治肢体缺血-再灌注损伤。

Abstract:Objective To observe the effect of aescin on lipid peroxidation of limb ischemia/reperfusion injury.Methods A total of 16cases were randomly divided into 2groups:the control group(n=20)and treatment group(n=20).Pa-tients in the treatment group were given aescin(16mg),while those in the control group had the same volume of bal-anced salt solution 10min before the onset of limb ischemic operation.Serum MDA,CPK,and SOD were observed respectively before limb ischemia,30,90,and 180min after the reperfusion.Results A significant serum MDA and CPK increase,and SOD decrease were seen at reperfusion 30,90,and 180min when compared with the preischemia values in the control group.In the treatment group the serum MDA and CPK values were significantly lower than those of the control group at reperfusion 30,90,and 180min(P0.01,P0.05respectively).SOD activity in-creased gradually in the treatment group and evidently higher than the value of the control group at 180min reperfusion(P0.05).Conclusion Aescin can reduce lipid peroxidation and the serum CPK level,enhance SOD activity,and effectively prevent the reperfusion injury of the limb ischmia.
