儿童幕下胶质瘤的磁共振灌注成像技术研究

摘    要:目的:研究磁共振灌注成像技术应用于儿童小脑幕下胶质瘤诊断的可行性及其扫描技术的优化。材料与方法:儿童小脑幕下胶质瘤患儿15例,年龄5-14岁,对照组25例,年龄3-14岁。使用GE1.5T HDx echo speedplus MRI扫描仪,双倍剂量造影剂马根维显(0.2mmol/kg)根据不同患儿血管情况分别以2~4ml/s注入速度通过24G套管针经最佳的血管注射,随后以相同速度注射生理盐水20ml。结果:磁共振灌注图像质量分为"差""中""优"分为三级,造影剂注射速度2ml/s组灌注图像质量多为"差"(12/17),注射速度3ml/s组多为"中"(14/17),注射速度4ml/s组全为"优"(6/6)。结论:磁共振灌注图像的质量与造影剂注射速度密切相关,造影剂注射速度3ml/s是获得满意磁共振灌注图像的基本条件;而扫描前护士对血管的选择及血管状况的评估是造影剂高速注射成功的前提。

Abstract:Purpose:To study the feasibility and optimization of Magnetic Resonance Perfusion Weighted Imaging(MR PWI)in children with infratentorial glioma.Materials and Methods:Children patients with infratentorial glioma(n=15)and without cerebral disease(n=25)were selected in this study,the age were ranged from 3 to 14 years old in the children.GE 1.5T HDx echo speed plus MRI scanner were used and doubling dosage(0.2 mmol/kg)Gd-DTPA was given by 24G trochar through the best vein of the child.The inject speed was ranged from 2~4ml/s according to the limit of the vein.Results:The image quality was divided into bad,good and best.The image quality was bad in the group which inject speed was 2ml/s (12/17),the image quality was good in the group which inject speed was 3ml/s (14/17) and the image quality was the best in the group which inject speed was 3ml/s (6/6).Conclusion:The image quality was determined by the inject speed of Gd-DTPA(3ml/s) and the inject speed of children patients was determined by burthen ability of the vein.
