多层螺旋CT及其后处理技术在舟状骨骨折中的应用

目的探讨多层螺旋CT及其后处理技术在舟状骨骨折中的临床应用价值。方法 26例舟状骨骨折患者经X线检查后,采用多层螺旋CT薄扫、三维重建及多平面重建,就X线平片与多层螺旋CT影像结果进行比较及综合分析。结果 X线平片漏诊2例结节部骨折及3例中远端1/3无移位裂隙骨折及1例月骨周围脱位,将2例裂开的舟状骨骨折误诊为无移位的裂隙骨折,此7例X线平片漏误诊的骨折均在多层螺旋CT予以确诊。结论多层螺旋CT及其后处理技术相较于X线平片而言在舟状骨骨折的诊断准确性上有较高价值,有利于临床选择治疗方案和制定手术计划。

Application of Multi-slice Spiral CT and the Imaging Postprocessing Technique in Scaphoid Fractures

Objective To evaluate the value of the application of multi-slice spiral CT and the imaging postprocessing technique in scaphoid fractures.Methods 26 cases were studied.After X-ray plain film,all cases were taken multi-slice spiral CT,three dimensional（3D） reconstruction and multi-planar reconstruction.Comparison and analyses of imaging appearance were performed between X-ray plain film and multi-slice spiral CT.Results Among 26 cases,2 cases of tubercle fracture,3 cases of extreme 1/3 fissure fracture and 1 case of lunatum luxation were missed on the X-ray plain films.2 cases of scaphoid fracture were misdiagnosed as non-shifted fissure fracture.These 7 cases were obviously demonstrated by multi-slice spiral CT.Conclusion Compared to X-ray,multi-slice spiral CT and the imaging postprocessing technique are valuable on making a correct diagnosis in scaphoid fractures.They are beneficial to choose treatment and decide the operative plan.
