超声心动图对混合型完全性肺静脉异位连接的诊断价值

目的：探讨混合型完全性肺静脉异位连接（TAPVC）的解剖类型、诊断方法及要点，以及超声心动图对本病的诊断价值。方法：近5年在我中心确诊为混合型TAPVC的患者，分析其主要的影像学检查结果。结果：17例混合型TAPVC患者有14例为心上+心内型，3例为心上+心下型。超声心动图发现5例肺静脉回流梗阻，而CT仅发现1例肺静脉回流梗阻。超声心动图准确探查到所有肺静脉回流径路者有7例，准确探查到3条者有4例。结论：混合型TAPVC以心上+心内型最多见。结合超声心动图和多排螺旋CT扫描是术前评估的最佳策略。超声心动图能准确显示大部分肺静脉回流径路，对显示肺静脉回流梗阻优于多排螺旋CT。

The diagnostic value of echocardiography on mixed total anomalous pulmonary venous connection

Objective： To study the anatomy variants and diagnostic methods for the mixed total anomalous pulmonary venous connection （TAPVC）, and the value of echocardiography as a diagnostic tool for this disease. Methods： To retrospectively analyze the main imaging data for those who were diagnosed as mixed TAPVC in our tertiary center in recent 5 years. Re- suits： Among the 17 mixed TAPVC patients, 14 were supracardiac plus cardiac, 3 were supracardiac plus infracardiae. Pul- monary venous obstruction as observed in 5 of them, all by echocardiography, and only 1 by multi-detector CT scan. E- chocardiography detected all pulmonary veins precisely in 7 patients, 3 pulmonary veins correctly but 1 incorrectly in 4 pa- tients. Conclusion： Supracardiac plus cardiac are the most common type of mixed TAPVC. The combination of eehocardiogra- phy and multi-detector CT scan are the best strategy for preoperative evaluation of these patients.
