X 线平片和CT 在脊柱结核诊断中的互补性

摘    要:目的 探讨平片和CT对脊柱结核不同的诊断价值。方法 收集40例脊柱结核病人的平片和CT资料,分别对受破坏椎体的检出率、椎体破坏的类型、附件及椎管内有无受侵犯和椎旁软组织情况分别统计。结果 平片对受累椎体的检出、椎间隙狭窄及后突畸形等方面比CT优越,而CT对脊柱结核破坏的类型、范围、附件和椎管内有无累及、软组织肿胀、冷脓肿和钙化等优于平片。结论 平片和CT结合能提高脊柱结核的诊断率。

Abstract:Objective To sum up the supplementary effects of X ray and CT in diognosis of vertebral tuberculosis. Methods The X ray and CT findings of 40 cases of spinal tuberculosis confirmed by operation and clinical data were analyzed.To evaluate the rate of vertebral destruction,the patterns of bone destruction,icidence of invelvement of vertebral appendages and paravertebral soft tissue masses. Results The X ray findings have significant value in the rate of vertebral destruction,intervertebral disk destruction,and deformity.CT is more sensitive in demonstration of the patterns of bone destruction,the range of vertebral involvement,involvement of vertebral posterior element and soft tissue masse in the epidural and paravertebral and calcification. Conclusion Rational use of various imaging modalities would improve the diagnosis accuracy of spinal tuberculosis.
