闭式胸膜活检对渗出性胸腔积液的诊断价值分析

目的 观察闭式胸膜活检术在渗出性胸腔积液中的诊断价值.方法 对104例渗出性胸腔积液患者行胸膜活检,同时取痰及胸水送检抗酸杆菌及癌细胞.结果 104例胸膜活检活检成功率78.8%,有特异性病理诊断62例,病理诊断阳性率59.6%.恶性胸腔积液胸膜活检阳性率58.3%,胸水细胞学检查阳性率22.2%,痰找癌细胞阳性率2.7%.结核性胸腔积液胸膜活检阳性率67.6%,痰找抗酸杆菌阳性率4.4%.结论 闭式胸膜活检不但是胸膜疾病内科确诊的一项重要手段,而且简单、安全、有效.

The diagnostic value of the closed pleural biopsy in exudative pleural effusions

Objective To observe the diagnostic value of closed pleural biopsy in exudative pleural effusions.Methods 104 cases patients with exudative pleural effusion were performed pleural biopsy,and the sputum and pleural effusion were sended to examine acid-fast bacilli and cancer cells at the same time.Results The achievement rate was 78.8% in all of 104 cases patients with pleural biopsy,there were 62 cases with specific pathological diagnosis,the pathological diagnosis positive rate was 59.6%.The pleural biopsy positive rate was 58.3% in Malignant pleural effusion.The positive rate of pleural fluid cytology was 22.2%,the sputum positive rate of 2.7%.Tuberculous pleural effusion pleural biopsy positive rate of 67.6%,and the sputum acid-fast bacilli positive rate was 4.4%.Conclusion Closed pleural biopsy is not only the way to diagnosis of pleural diseases in internal medicine,it is also the simple,safe and effective means.
