低分子肝素与曲美他嗪治疗不稳定型心绞痛临床观察

目的观察低分子肝素与曲美他嗪治疗不稳定型心绞痛(UAP)的疗效。方法86例UAP患者随机分为2组,其中对照组42例,治疗组44例;2组均常规给予硝酸酯类、β-受体阻制剂及阿司匹林等;治疗组给予低分子肝素钠4000U,每日2次,皮下注射,连用5~7d,加用曲美他嗪20mg,每日3次,连用15d;对照组给予普通肝素钠5000U,每日2次,皮下注射,连用5~7d,观察比较2组临床疗效及心电图改变情况。结果治疗组临床总有效率为90.91%,对照组总有效率为69.05%,治疗组明显优于对照组(P<0.01)。治疗后治疗组ST段下降导联数(NST)、ST段下降的数值总和(∑ST)及心肌缺血总负荷均较治疗前显著改善(P<0.01)。治疗组有5例皮下瘀斑,2例进展为AMI;而对照组有8例进展为AMI,均行冠脉介入治疗。结论低分子肝素联用曲美他嗪治疗UAP安全有效。

Clinical observation of low molecular weight heparin combined with trimetazidine on the treatment of unstable angina pectoris

Objective To observe the effect of low molecular weight heparin combined with trimetazidine on the treatment of unstable angina pectoris (UAP).Methods Eighty-six patients with UAP were divided into two groups.There were 42 patients in the treatment group and 44 patients in the control group. All of them were given the nitroglycerin, beta-blockers and aspirin etc.Treatment group were used the low molecular weight heparin 4 000 IU,twice a day hypodermic,after five to seven days,added the trimetazidine 20 mg,three times a day,it was used 15 days.The control group was used heparin sodium 5 000 IU,twice a day hypodermic,it was used five to seven days.Results The total effective rate of the treatment group was 90.91%, and the control group was 69.05%,the treatment group was much better than the control group (P<0.01).The treatment group contained 5 patients cases of subcutaneous purpura, and only 2 patients with AMI, but the control group has 8 patients with AMI.All the patients were treated with coronary interventional therapy.Conclusion The low molecular weight heparin combined with trimetazidine is more safety and effectively on UAP
